<br />CDBG FUNDiNG REQUEST APPLICATION <br /> <br />I. GENERAL INFORMATION <br />A. Proposed Activity <br />Arc is requesting $1,000 from the City of Centerville CDBG funds. <br />Funds will be used to support the most essential services offered <br />to residents of Anoka County and specifically the City of <br />Centerville. <br /> <br />These services encompass a wide range of activities including <br />leisure services, information and referral services; one-to-one <br />advocacy and problem solving with adults with disabilities and <br />the Toy and Adaptive Equipment Lending Library. Arc has over 650 <br />members in Anoka County and provides direct services to an <br />estimated 900 to 1,000 people in the county. Arc provides <br />information and direct serve to approximately 50 people from the <br />City of Centerville. <br /> <br />Arc has provided these services to Anoka County and the City of <br />Centerville residents for over ten years and would welcome the <br />continued support of the City through CDBG funding. <br /> <br />Community Needs Addressed by this Project: <br /> <br />Adult services: Over the years, services for adults with <br />developmental disabilities have focused on assisting people to be <br />productive, participating members of their home communities. <br />Emphasis has shifted from large institutional residential <br />services to typical community homes serving four to six people, <br />and supported employment in typical jobs rather than large <br />sheltered workshops serving only people with disabilities. This <br />change has proven to be successful - adults with mild to moderate <br />disabilities are participating in their communities and working <br />in jobs that contribute to the community and adults with more <br />severe disabilities are finding opportunities to be more <br />involved. <br /> <br />Arc has been instrumental in supporting legislation to make these <br />changes possible and providing one on one assistance to adults <br />with disabilities and their families to access the services <br />available. Arc provides information and support to adults living <br />in group homes and individuals living in your city. For these <br />individuals, community participation is a personal goal and also <br />a challenge given the limits that a developmental disability <br />places on a person. <br /> <br />Through the Arc individual advocacy program, People First Group <br />and information and referral services Arc provides one on one <br />support to adults with developmental disabilities to help: <br />identify supports and services to live successfully in the <br />community; apply for services and complete intake processes; <br />resolve problems and issues with services; identify and access <br />
