TTY 783-4724 <br />. <br /> <br />A United Way <br />Agency <br /> <br />January 12, 2000 <br /> <br />City of Centerville <br />15544 Central Avenue <br />Centerville, MN 55304 <br /> <br />Dear City Council Members of Centerville: <br /> <br />Enclosed please find the CDBG funding proposal on behalf of the <br />Centerville Senior Outreach Program, administered through Anoka <br />County Community Action Program. <br /> <br />Phyllis Linder continues to serve on as the Senior Outreach <br />Worker for City of Centerville. Through. your support she is able <br />to visit seniors and disabled adults in your community, providing <br />case management, referral and advocacy services for your <br />residents. She helps seniors and disabled adults by assisting <br />them in obtaining help through food programs, health services, <br />energy assistance, and crisis home repairs, etc. Without her work <br />in the community, many of these individuals would not get the <br />help they need, left in their homes with inadequate food, medical <br />care, or other basic necessities. <br /> <br />Because CDBG dollars are in such demand, some other cities are <br />funding the Senior Outreach Program through revenue from cable <br />television or through city dollars.
