How long has project been going on? <br /> <br />The Senior Outreach Program has been in existence for <br />over ten years. <br /> <br />2. Detail past activities by year <br /> <br />The Senior Outreach Worker provided services <br />throughout the past year for clients. She assisted <br />them when applying for Energy Assistance, or other <br />programs/services. (See project description for a <br />more detailed explanation of outreach services). <br /> <br />3. Detail future activities by year. <br /> <br />If CDBG funds are allocated, the Outreach Worker will <br />continue to serve the community with complete <br />outreach services. If no CDBG funds are allocated, <br />then outreach services will be limited to Energy <br />Assistance applications and crisis intervention. <br /> <br />D.
