<br />Premises Permit Application - LG214, Instructions <br /> <br />1/99 <br /> <br />Attachments <br /> <br />1. Lease and sketch. <br />If the gambling premises is leased, attach a copy of: <br />· the appropriate lawful gambling lease form, and <br />· the sketch of all leased areas with dimensions and square footage <br />clearly defined. <br /> <br />Lease forms: <br />LG221 - Lease for pull-tab, paddlewheels, tipboard, and/or raffle activity. <br />LG222 - Lease for bingo activity. <br />LG223 - Lease for pull-tab dispensing device activity. <br /> <br />2. Permit Fee. <br />For each premises permit application, attach a separate check, payable <br />to the State of Minnesota, for the appropriate permit fee: <br />Class A - $400 <br />Class B - $250 <br />Class C - $200 <br />Class 0 - $150 <br /> <br />3. Resolution of Approval. <br />· The premises permit application must be approved by the local unit of <br />government (city or county). Attach a copy of the resolution which <br />shows approval of your application. <br /> <br />· If the local unit of government denies your application, do not submit <br />the application to the Gambling Control Board. <br /> <br />Where to mail your application Mail your completed application with the attachments to: <br />Gambling Control Board <br />Suite 300 South <br />1711 West County Road B <br />Roseville, MN 55113 <br /> <br />Processing Allow ten weeks for processing the application. Your organization's chief <br />executive officer will be contacted in writing regarding any missing information. <br /> <br />Term of permit The expiration date of a premises permit is the same as the expiration date of <br />your organization's license. <br /> <br />Local ordinances Check with the local unit of government (city or county) regarding local <br />ordinances, if any, that apply to lawful gambling. Local ordinances may be <br />more restrictive than statute or rule. <br />
