<br />Since injuries at skate parks can be severe, it is necessary that minimum medical standards be met <br />Besides having your personnel trained in CPR and first aid, you should have the following readily <br />available: <br />. A telephone or radio communications to emergency services; <br />. A complete medial first-aid kit on premises; <br />. Ice packs and sanitary towels available; and <br />. An accident report to be completed by facility supervisor on duty following any accident or <br />injury occurring at the facility. <br /> <br />A complete accident report should be filed by the employee on duty following any accident or <br />injury occurring at the skate park. All accident reports must be kept on file by the city. This report <br />should include: <br /> <br />1. Date; <br />2. Time of day; <br />3. Injured person's name, address and phone number, <br />4. If the injured person is a minor, the name ofhislher parent or guardian; <br />5. Names and phone numbers of any witnesses; and <br />6. A complete description of the events and circumstances surrounding the accident or injwy. <br /> <br />Faeility roles, regulations, and sigDage <br />An aggressive ska1ing facility must be signed and constructed with the priority of reducing the risk <br />of injury to participants and spectators. Written facility rules and regulations must be posted in a <br />visible area. Signage should be easy to read and must be accompanied by visual symbols, and <br />wherever necessary, be translated into the language of the community. <br /> <br />The facility will need to post and enforce established rules and regulations to include, but not <br />necessarily limited to: <br /> <br />. Operating hours and admission requirements <br />. Possible age and skill level requirements <br />. Behavior requirements <br />. Requirements regarding the use of personal protective and safety equipment <br />· Requirements concerning the fitness for use of skateboards and related equipment <br />. Skate runs should be labeled clearly as to degree of difficulty <br />. In addition, the park should retain the right to revoke skating privileges of participants who <br />are rowdy or who do not abide by the rules <br /> <br />All users of a skating facility need to complete the following: <br /> <br />. An understanding agreement of the rules and regulation of the park. (Attachment A) <br />· A facility registration card before using the facility. Registration cards must be kept on <br />premises. (Attachment B) <br />· A facility identification card that will be shown to gain entrance to the facility. The user must <br />carry an identification. (AttaC'.hment C) <br />. A warning and waiver signed by the skater and guardia14 if a minor. (Attachment D) <br />
