<br />Additional BMP Summary Sheet <br />MS4 Name: City ofCenterville <br />Minimum Control Measure: N/A <br />Unique 8MP Identification Number: IX-C.H <br /> <br />8MP Title: Discharges Affecting Source Water Protection Areas <br />8MP Description: <br />BMP 7-1: Discharge Affecting Source Water Protection Areas <br />The Minnesota Department of Health (MDH) has not yet approved a Part I and Part II Wellhead Protection Plan <br />(WPP) for the City including the necessary map illustrating the wells and source waters for drinking water supply <br />management areas identified as vulnerable under Minn. R. 4720.5205, 4720.5210, and 4720.5330. When the plan <br />has been completed according to MDH timeline. The City will revise the SWPPP as appropriate. <br />Centerville will determine if the City jurisdictional area includes land within the source water protection area for <br />surface water intakes identified in the source water assessments conducted by the Minnesota Department of Health <br />under the federal Safe Drinking Water Act, U.S.C. 300j-13. [fso, the City will work with the appropriate <br />organizations to address potential impacts to the areas to the MEP and revise the SWPPP accordingly. <br />Centerville will rely on the guidance document from the MN Dept. of Health titled "Evaluating Proposed <br />Stormwater Infiltration Projects in Vulnerable Wellhead Protection Areas" in determining the appropriate locations <br />for stormwater infiltration devices, BMPs, and structures. <br />Location(s) in SWPPP of detailed information relating to this 8MP: <br />The information included or referenced on this Summary Sheet is intended to meet all SWPPP requirements for this <br />BMP. <br />Measurable Goals: <br />. Develop Wellhead Protection Plan as approved by MDH. <br />.
