<br />The actual amount of time off, and funeral leave approved will be <br />determined by the department head depending on individual <br />circumstances, (such as the closeness of the relative, arrangements to be <br />made, distance to the funeral, etc.). In these situations, employees will <br />receive their regular pay except that employees will not be paid for <br />regularly scheduled days off. <br /> <br />11.4 Parental Leave <br /> <br />Eligibility for Parental Leave shall be in accordance with the Family and <br />Medical Leave Act and the Minnesota Parenting Leave Act. <br /> <br />12 INSURANCE PLANS <br /> <br />12.1 Health Insurance <br /> <br />Upon proper application and subject to any required premiums, all regular <br />full time employees will be required to be covered by the City's health <br />insurance plans. Subject to the payment of the required premiums, an <br />employee's dependents may also be covered under such plans. <br /> <br />a) The City Council will periodically review the percentage <br />of premiums the City pays and will determine whether a <br />change in the amount is warranted. <br /> <br />b) The balance of the monthly premium will be paid by the <br />employee through authorized payroll deductions. <br /> <br />c) Any employee leaving City service for any reason may <br />continue coverage under the City's group insurance <br />plan for a period not to exceed Federal or State <br />requirements. Such employees must pay the entire <br />premium for that coverage and may not upgrade the <br />coverage from single to family, unless otherwise <br />required by law. <br /> <br />12.2 Dental and Life Insurance <br /> <br />Upon proper application and subject to any required premiums, all regular <br />full-time employees will be covered by the City's dental and life insurance <br />plans.
