<br />a. Health insurance premiums spending account; <br />b. Un-reimbursed health care spending account for items not eligible <br />for coverage in health coverage and H.S.A.; <br />c. Day care spending account; <br />d. Deferred Compensation account. <br /> <br />5. The dollar limit for employee contribution to the un-reimbursed health <br />care spending account shall be set at: <br />a. $3,000 <br /> <br />AND BE IT FINALLY RESOLVED, AS FOLLOWS: <br /> <br />1. That there is hereby established the Wells Fargo Employer HSA. <br />2. That the Council approves a high-deductible health coverage plan through Health <br />Partners. <br />3. That the mayor, administrator and finance director are hereby authorized to <br />execute necessary documents to create and administer the program of health <br />insurance and employee health savings accounts. <br /> <br />Adoption ofthis resolution amends the City of Centerville Employees - Cafeteria Plan dated <br />January 8,2003. <br /> <br />Adopted by the Council this 30th day of December, 2008. <br /> <br />Mayor <br /> <br />Attest: <br /> <br />City Clerk <br /> <br />44 <br />
