<br />2. Regular Unpaid Leave of Absence. <br />The City Administrator may grant an unpaid leave <br />of absence to any regular employee upon written <br />request for a period not exceeding 90 days for <br />career advancement, or for personal or family <br />situations. Under no circumstances may an employee <br />use a leave of absence to work for another <br />employer or to pursue self -employment. Prior to <br />requesting a leave under this section, an employee <br />must first exhaust all available paid leave. The <br />employee must request a leave of absence in <br />writing and forward it to their immediate <br />supervisor, who will then direct the request to <br />the department head or the City Administrator with <br />the supervisors/department head's recommendation. <br />An unpaid leave of absence requires the advance <br />written approval of the City Administrator. The <br />City Council may extend such leave to a maximum <br />period of one year if the employee is disabled or <br />where the City Council finds extraordinary <br />circumstances warrant such extension. Benefits <br />including vacation leave, sick leave, holidays or <br />other forms of indirect compensation will not <br />accrue during an an unpaid leave of absence. The <br />employee may continue to be covered by group <br />health, life and disability insurance, but will be <br />responsible for paying one hundred percent (100%) <br />of the premium costs. During the first calendar <br />month, the employee will be responsible for paying <br />the pro-rated portion of their monthly insurance <br />premiums for the days they are on on unpaid leave. <br />Subsequent payments are due to the City by the <br />15th of each month. <br /> <br />K. Absence Without Leave. An employee who is absent and <br />fails to report the reason to the employee's supervisor <br />wi thin one hour following the start of the employee's <br />workday shall be subject to discipline, which may <br />include deduction of pay for the period of absence. <br /> <br />V-13 <br />
