<br />4. Licensed Premises. The license is only effective for the compact and contiguous <br />space specified in the approved license application. If the licensed premises is <br />enlarged, altered, or extended, the licensee must inform the Director of Public <br />Safety. <br /> <br />5. Effect of License Suspension or Revocation. It is unlawful to solicit business or <br />offer to perform tattooing, body piercing, body branding or body painting services <br />while under license suspension or revocation by the City. <br /> <br />6. Maintenance of Order. The licensee is responsible for the conduct of the business <br />being operated and must at all times maintain conditions of good order. <br /> <br />7. Employee Lists. The licensee must provide to the City Administrator a list of <br />employees who perform tattooing, body piercing, body branding or body painting <br />at the licensed establishment and verify that each employee has received a copy of <br />this Article. <br /> <br />8. Liability Insurance. Licensees must have at all times a valid certificate of <br />insurance issued by an insurance company licensed to do business in the State <br />indicating that the licensee is currently covered in the licensed business by a <br />liability insurance policy. The insurance must be kept in force during the ternl of <br />the license and provide for notification to the City prior to termination or <br />cancellation. A certificate of insurance must be filed with the City. The <br />minimum limits of coverage for such insurance are: <br /> <br />" <br />A. <br />/7. <---.. B. <br />~~J~EALTH AND SANITATION REQUIREMENTS <br /> <br />Each claim, at least $200,000; <br />Each group of claims, at least $500,000. <br /> <br />Unlawful Practice. It shall be unlawful to engage in the practice of tattooing, body <br />piercing, body branding or body painting at any place in the City without complying with <br />the regulations ofthis Ordinance. <br /> <br />Lavatory Requirement. A place where tattooing, body piercing, body branding or body <br />painting is practiced must be equipped with an adequate and conveniently located toilet <br />room and hand lavatory for the accommodation of employees and patrons. The hand <br />lavatory must be equipped with hot and cold running water under pressure, be maintained <br />in good repair at all times and kept in a clean and sanitary condition. Toilet fixtures and <br />seats must be of a sanitary open front design and readily cleanable. Easily cleanable, <br />covered receptacles must be provided for waste materials. A lavatory facility must be <br />provided with an adequate supply of hand cleansing compound and singe-service sanitary <br />towels or hand-drying devices. <br /> <br />Skin Infection. A person having a skin infection or other disease of the skin may not be <br />tattooed, painted, or branded. <br /> <br />Page 5 of 12 <br />
