<br />M:.e"'ll1" <br />i: "0' <br /> <br />/ " ..'-' __i,.:,; 0':' '.'; <br /> <br />TO: Honorable Mayor and Council Members <br /> <br />FROM: Teresa Bender, City Clerk~'~' <br /> <br />SUBJECT: Center Mart - Provisional Tobacco License (Expires July 27, 2004) <br /> <br />DATE: July 9, 2004 <br /> <br />Mr. Carl Buechler was granted a provisional license to sell tobacco products at retail for a <br />period of two (2) months on May 26, 2004 to allow time for a background check and <br />submission of insurance certificates. <br /> <br />Center Mart's anticipated opening was July 2, 2004 and Mr. Buechler provided the City <br />with the attached documentation (General Liability Insurance and a hand-written binder <br />for Worker's Compensation Insurance), Mr. Buechler was instructed to supply the City <br />with a copy of the binder on Tuesday, July 5, 2004. The background check has been <br />completed by the Centennial Lakes Police Department. Acting Mayor Broussard Vickers <br />and myself felt that Mr. Buechler had satisfied the Council's requirements and issued the <br />attached provisional license. <br /> <br />However, on July 5, 2004 I received the attached fax. Staff has a concern that Mr. <br />Buechler does not have Worker's Compensation Insurance and may not have it by the <br />expiration date ofJuly 27, 2004. <br /> <br />Staff requests that Council consider granting Center Mart's License to Sell Tobacco <br />Products at Retail from July 27 through December 31,2004 subject to the submission of <br />their Worker's Compensation Insurance Certificate with the clear understanding that NO <br />license will be issued until this requirement is met. <br />
