<br />1. No person shall provide ~assage theraPy
<br />without being Ucensed individually as provided
<br />in this chapter,
<br />2_. Applicatlon for license. MY person desir-
<br />'ing to be licensed shall file an application on
<br />forms provided by the Cily Adminlstrator. The
<br />application $hall contain .such Infonna'lon as
<br />the City Administrator may require. including:
<br />(A) The applicant's full name, address, social
<br />security number, and written proof of age.
<br />(B) The name and address of the establisf1.
<br />ment by which the appll-cant expects 10 be
<br />employed.
<br />(C) f1:. statement conceming whether the per-
<br />son has been convicted of or entered a plea at
<br />. guilty to any crime or ordinance violation and,
<br />if so, information as to the time. place and
<br />. nature of such Clime or offense.
<br />{D} Proof thallhe applicant meets lhe follow-
<br />Ing educational requirements:
<br />0) A diploma or certificate of graduation from
<br />a school approved by Ihe American Massage
<br />Therapist AssociatIon 0( other similar reputable
<br />massage a.ssociatlon; or
<br />(Jij A diploma or certificate of graduation from
<br />a school which is sithe\' ac",redited by a
<br />recognized educational accrediting association
<br />. or agency, or is licensed by the St~e or local
<br />government agency having jurisdiction over tM
<br />school.
<br />(iii) Each applicant shall also furnish proof at
<br />the time of application of a minimum of six hun-
<br />dred (600) hours of succeSSfully completed
<br />course work in the following areas:
<br />a) The theary and practice of massage, in-
<br />cluding, but not limited to, Swedish, Esalen.
<br />Shiatsu, and/or Foot Reflexology techniques;
<br />and .
<br />b) A.natomy, including, but not jimited t~.
<br />skeletal and muscular structure and organ
<br />placement; and
<br />c) Hygier\e.

<br />1. No person, partnership 01 corporation shall
<br />engage 11'1 a business. which has massage
<br />therapy as an accessory use, without being
<br />licensed as provided In this chapter. '
<br />2. The initial application for a licel'lse bY a
<br />business shall be made by completing an ap-
<br />plication form provided by and containing such
<br />information as required by the City Ad-
<br />ministrator and by paying-. ol'l9tlrne nonrefun-
<br />dable initial application fee of $100.00.
<br />3. If the application for a license under this
<br />cJ:lapter is granted, the $100_00 fee above shall
<br />be the first year's license fee. Thereafter, the
<br />annual license fee shall be $100,00. A ~te
<br />license shall be abtained tor each pface~ of
<br />business. .
