If the partnership is required to file a certificate for a trade name under state law, a copy of that certificate must also be attached. d. if the applicant is a corporation or other organization: (1) the name; and if incorporated, the state of incorporation; (2) a true copy of the certificate of incorporation, articles of incorporation or association agreement, and by-laws.
