Following is a listing of the requirements: <br /> <br />1. Personal Service (Massage) Application For A New License needs to be <br />filled out. <br /> <br />2. Three letters of reference - Page 2. <br /> <br />3. Release of Information form - signed. <br /> <br />4. Provide evidence of educational qualifications - copies of degrees, <br />diplomas or certificates, if any. <br /> <br />5. Copy of your drivers license. <br /> <br />6. Show proof of insurance coverage of one million dollars for personal <br />liability in the practice of therapeutic massage. <br /> <br />., <br /> <br />7. The fee is $50.00. <br /> <br />A completed application form and all of the following information is required for the <br />Police Department investigation.
