This renewal is effective February 1, 2005 through January 31, 2007. The product offering does reflect a change in your rate and product offering. The product is designed for customers with less than 200 employees. Fac<Ho-F ace Assistance 1-3 _slons with Full-Service Ufe Events $5,100 per year (billed quarterly) Although onsite employer service hours are excluded from your current rate, you may elect to purchase a block of 3 onsite hours for $600 per year if you are interested. These hours can be used to provide wellness seminars, critical incident response, supervisory training, and other on-site services for your employees and managers. We also offer free monthly telephonic seminars you can participate in the day of the seminar. If you or your employees are not able to participate, the audio may be accessed by calling the designated number provided to you up to five days after the seminar is conducted. In addition, CIGNA Behavioral Health offers a wealth of information available through ourwebsite. You and your employees will be able to access many of the same convenient, practical, and confidential tools and information available telephonically. Our inleractive site empowers participants with lools 10 support their varied needs and interests. In addition to the interactive tools, self-assessments and resource material, your employees can also find a provider and access a referral online at their convenience.
