<br />Centerville City Council <br /> <br />2 <br /> <br />April 4, 2005 <br />PWSID: 1020036 <br /> <br />. You can distribute a copy of the report to all of your customers by mail or other means. Efforts must <br />be made to get the reports to actual customers rather than to just the billing customer. One method is <br />to mail the report to all postal patrons within the service area using a "Simplified Address"; check with <br />your post office about this. Other options include publicizing the availability of the report through the <br />media, delivering multiple copies for distribution by single-biller customers such as apartment <br />buildings or large private employers, and posting the report on the internet. <br /> <br />. You can also satisfy the distribution requirement by publishing the report in one or more local <br />newspapers serving your service area and inform your customers that the report will not be mailed to <br />them but is available upon request. If you use this option, the entire report must be published (not <br />just a summary), and you must send a copy or clipping of the newspaper report, along with the <br />Certification of Completion form. <br /> <br />A copy of the completed report must be submitted to the MDH, along with a Certification of Completion <br />form certifying that the information in the report is accurate and that the required distribution to customers <br />was completed. You must also retain a copy of the report for at least three years. Failure to produce <br />and distribute a report as required--as well as failure to submit the required documentation to <br />MDH by July 1, 2005--may result in enforcement actions, Including fines, from the U.
