A. Now does the project benefit low /moderate residents of Anoka <br /> County: <br /> This project serves adults with developmental disabilities in <br /> Anoka County. Approximately 100% of adults with developmental <br /> disabilities are low or moderate income. Many work in sheltered <br /> or supported work environments for sub - minimum wage or minimum <br /> wage. Few have full time employment with benefits. Total adults <br /> with disabilities served in the City of Centerville is 5 -10. <br /> Families who use the adaptive equipment and toy lending library <br /> are generally low income and do not have the resources to <br /> purchase toys for their young children. 50% of the families who <br /> use the Library are low or moderate income. Approximately 3 -5 <br /> families from the City of Centerville will use the Toy and <br /> Adaptive Equipment Lending Library. <br /> Income is verified by: Special needs population presumed to be <br /> low /moderate income, and information supplied by families as they <br /> apply to use the Library. <br /> B. Process <br /> For adults with developmental disabilities the advocate will <br /> verify that the individual is either receiving case management <br /> services through Anoka County Developmental Disabilities in which <br /> case the County has verified eligibility or the advocate will <br /> work with the individual to verify their disability and apply for <br /> appropriate services. <br /> For families applying to the Library, the Family Services <br /> Director will ask for information on the application form. No <br /> further investigation is performed. <br /> C. The offices are located in the Anoka County Human Services <br /> Center, 1201 89 Ave NE #305, Blaine. Services are provide <br /> throughout the community including cities and areas considered <br /> to be low /moderate project areas. <br /> V. Project Duration: <br /> The services are ongoing and have received past CDBG Funding. <br /> The project has served families through the Toy and Equipment <br /> Lending Library and adults through the advocacy and self advocacy <br /> program. Services have been provided in Anoka County since the <br /> mid 1980's. The program is expected to continue in the future. <br /> 5 <br />
