Dear Mr. and Mrs. Hasiak,
On June 17� 2014,your dog, a German Shepard,was involved in a bite incident(Centennial
Lakes Police Department ICR#MN0020300).You are hereby notified pursuant to City
Ordinance that your dog is designated a POTENTIALLY DANGEROUS ANIMAL.
Pursuant to City Ordinance, an owner of a potentially dangerous animal shall be required to
annually register the dog with the City as a potentially dangerous animal and provide proof of
vaccinations to the city. The owner shall provide and maintain a proper enclosure.Also,the dog
must be muzzled,restrained by a substantial chain or leash(not to exceed 6 feet in length), and
under the physical restraint of a person 16 years of age or older if outside the proper enclosure.
Pursuant to City Ordinance,you must obtain microchip identification for your dog. The name of
the manufacturer and identification number of the microchip must be provided to the Community
Service Officer. Please make arrangements to register and microchip your dog within two(2)
weeks.
Our Community Service Officer has provided you with a check list of the Potentially Dangerous
Dog Requirements for the cities of Circle Pines,Lexington, and Centerville.Please read and
understand these requirements.If you wish to appeal the designation,the form must be sent to
the Centennial Lakes Police Department and to City Hall.A Community Service Officer will
follow up within two weeks to make sure you have completed the requirements.

Potentially Dangerous Dog Requirements

1. The owner shall provide and maintain a proper enclosure.
2. If the dog is outside the proper enclosure the dog must be muzzled, restrained by

City and be issued a Certificate of Registration stating that the animal is
potentially dangerous.
4. The owner must provide and show proof of microchip identification as required
by state law. The name of the microchip manufacturer and identification number
of the microchip must also be provided to the Animal Control Officer.
***The Centennial Lakes Police Department may seize any dangerous or potentially
dangerous dog if the owner does not meet each of the above conditions within 14
(fourteen) days.***
If you intend to appeal this matter to your city council, please sign and return this form
to the Centennial Lakes Police Department within 14 (fourteen) days. You will want
to contact your city office to have this matter placed on the next City Council's meeting
agenda.

On 0,6-17-2014 at 1125 hours, I was dispatched to Meadow lane and Centerville road, on a dog bite.
Upon arrival, I met with the victim,JERRY CONRAD ALBRECHT(DOB09-24-1943)who said he was biking by 6994
Centerville road when a large German Shepherd came out from behind a fence and bit him on his right upper thigh.
ALBRECHT denied medical attention.ALBRECHT showed me the wound which was two small puncture holes. ALBRECHT
said he bikes past the house frequently,and never sees the dogs but he has heard them bark a lot in the past.
I then met with the owners of the dogs, STAN HASIAK(DOB:04-08-1950)and STEPHANIE SINAIDA HASIAK(DOB:12-05-
1952)both parties reside at 6994 Centerville road. STAN said he was outside in his backyard moving his two german
shepherds, HEIDI and SZARYK,from the backyard into their kennels downstairs. STAN said he forgot to close the gate near
Centerville road and HEIDI got out an ran toward Centerville road and ALBRECHT. STAN said SZARYK followed HEIDI who
was chasing ALBRECHT at this time. STAN said he saw SZARYK bite ALBRECHT in his leg, and took control of both dogs
after the bite.
STEPHANIE showed me current rabies vaccination(until 09-09-2016)for SZARYK. STEPHANIE also showed me their current
set-up for containing HEIDI and SZARYK in the backyard. I explained the quarantine period to both HASIAK's, and both agreed
to keep SZARYK under quarantine for the next 10-days(06-27-2014).SZARYK quarantined at 6994 Centerville road until 06-
27-2014.
1 took pictures of ALBRECHT's wounds, informed ALBRECHT that the dogs both had current rabies vaccinations, and told
ALBRECHT to contact the police department if he chooses to get any medical attention or if he has any questions.
Quarantine form completed, and pictures uploaded to CLPD server.
Pending follow-up.

ON 06-28-2014 AT 1006 HOURS, I CONDUCTED A FOLLOW UP AT 6994 CENTERVILLE ROAD,AFTER A 10-DAY
LU QUARANTINE PERIOD ENDED ON A DOG BITE.
UPON ARRIVAL, I MET WITH STEPHANIE HASIAK. I EXPLAINED TO HASIAK SZARYK WOULD BE LISTED AS
POTENTIALLY DANGEROUS, EXPLAINED THE REQUIREMENTS TO HER,AND THE APPEAL PROCESS.

MICROCHIP INFO ON MONDAY(06-30-2014),AND FIGURE OUT WHEN SHE WOULD BE ABLE TO

Thanks for talking with me on the phone today. I have always appreciated your candor and willingness to
work together.
Here is my proposal for your approval. That you will agree to voluntarily surrender your liquor licenses for the
Trio Inn on the condition that the City issue you a prorated refund for the application fee you paid for the
City's licensure. We both understand that this agreement does not include a refund for any license or
application fees for State licensure. However,voluntary surrender of your license could result in some refund
by State agencies should you work directly with them. That you also agree to allow this question to be decided
by the City Council on July 9, 2014, even though you cannot attend that meeting.

As I understand that you are currently out of the State,please respond this email to indicate your willingness
to join in this agreement.

I spoke to Paul this morning. He will agree to relinquish his liquor license on the condition that he is given a pro-rata
refund of the application fee he paid to the City. I told him I needed your authorization before proceeding.
Our code, referencing state law, can allow a refund for Paul under either of two conditions: the licensee ceases to carry
on the licensed establishment,or,the business ceases for a lawful reason other than revocation.
Paul now understands the City has grounds to revoke his license (i.e. non-payment of taxes,or cessation of business
activity), and understands that we cannot give him a refund once we revoke the license. Paul is not currently in the State
and cannot appear on Wednesday. He will agree (via email)to the terms stated above, for consideration and action at
Wednesday's meeting. Can I formally extend this offer to him?

Minutes of Work Session
Present were Mayor Tom Wilharber, Council members Jeff Paar, Steve King, D Love,
and Ben Fehrenbacher. Also present were Mark Statz of Stantec., and Finance Director
Ellie Paulseth.
The purpose of the work session was to cover three agenda items: Discuss engaging

review the 2015 Street Mill-Overlay Program.
Recruiter. At the previous interviews, Council consensus was that the firm of
Brimeyer-Fursman was preferred over Springsted. Mr. Fursman was invited to this
evening's meeting,but was unable to attend due to being out of the country. Council
considered whether a recruiting firm is needed. Mr. Fursman will be invited to the
Council meeting of July 23,to discuss the recruiting process. Administrator Larson
will discuss the issue with Fursman and see if it is possible to streamline the process
to reduce the cost.
Budget 2014. Finance Director, Ellie Paulseth, reviewed a draft budget for 2015. The
budget includes an increase in local government aid of approximately 10% from
$54,000, to $60,466. The expense budget includes $200,000 for the street overlay
program, and a significant increase in the amount allocated to Centennial Lakes
Police. The police budget has not been vetted by the Governing Board, so it is likely
that number will be reduced. The levy for Bonds increased slightly and the overall
levy would rise by 4.6%. No major increases are needed in utility rates,but an
inflationary adjustment may be indicated as well as an adjustment to Sewer rates to
account for Met Council's increase in sewer treatment cost. Storm Sewer rates may
need to increase to meet the needs for ditch and pond cleaning. Council asked that
rates be surveyed in neighboring communities for comparison as increases are
considered.
2015 Street Program. The Council reviewed the street mill and overlay program
presented by staff. The street list shows a maintenance project for 2015,of about
$278,000. The city budget contribution was increased from $169,000 to $200,000 and
special assessments increased to $525 per property to raise approximately$80,000 in
total assessments.
Administrator Larson reviewed projections for the program out through the year
2020. In the projections, City contribution is increased about $10,000 each year and
special assessments are increased by$25 each year per property. The cash flow works
well until 2018 when it goes into a deficit. However it appears that the deficit can be
managed by modifying the timing of the program, either deferring a few streets or
suspending the program for a year. Since the program in 2018 is overlaying streets
that are 11 or 12 years old, it seems very feasible to adjust to available cash flow.

vehicle have access to all homes in the neighborhood,that they make sure that IDs are checked with any dispensing of
