He had Soundfall backout due to health issues and a band member moving out of state. Mr. Sorensen hoped to be back in the future. Committee Member Kubat was going to work with the same individual to see if they would be willing to offer their services with all other parks. It may also be a good idea to work with Mr. Frevel at the same time. Staff is looking for direction or an update on this item. See attached HSP map for locations. Note: Park sign will need new City of Centerville logo when new logo is rolled out. Royal Meadows • Wood chips needed beneath swings • Basketball hoop needs new net • One dead tree (SW corner of park. See attached map for location.) Notes: This park has no ADA play equipment. This park has no bike rack. Trailside Park • Brush and trimmed branch pile should be removed. South edge of park. • Two dead trees on North side of park could be removed. • Stump near lake shore, middle of park should be "prepped and grass seeded". See attached map for locations. Cornerstone Park • 3 areas of dead bushes should be replaced 4 See attached map for locations. • Plexi-glass on west side of kiosk needs washing (can't read the enclosed info, may need replacing if washing doesn't clear it up) • Kiosk frame could use re -painting Centerville Park Maintenance Review: May 2021 page 2 of 3 McBride Park • Park sign needs prep and painting • Basketball hoop needs new net • Small pothole in trail needs re -filling to prevent further trail damage Notes: Play equipment looks OK to me. May need some paint in 2022. Adding a couple of pine trees along the East border would be nice. Eagle Park • Wood chips need to be added beneath the swings • There are quite a number of pine trees along the north side of the trail that could use trimming • (Trimming would benefit the overall health of the trees, as well as prevent "misbehavior") Note: Acorn Park The tennis/pickleball court looked in good shape. The Park sign wasn't too bad, but if McBride's sign is being painted .... 0 • Park sign needs prep and painting • The Prickly weed that was a problem in 2020 is re-emerging (nasty stuff) • Wood chips need replenishing beneath the swings and some of the play equipment • The metal gridded top of the picnic table is warping • Doggie pick up bag dispenser needs refilling • One pine tree along the west border is dead and should be replaced • Trim poplar/sumac brush from the base of the pine trees along the East border • Add oak trees. Notes: • The neighbor on the west side of Acorn Park requested that the Port -a -Potty be moved to the NW corner because of the offensive odor during the hot months of the summer. With the new parking area that has recently been added, the NW corner of Acorn Park looked to me like a good location for maintenance and Port -a -Potty cleaning purposes. • 1 would like to recommend that we accommodate this request as soon as the parking area is finished. • Please remind anyone mowing at Acorn that the Pollinator Garden was completed on May 23rd It is located in the SW corner of the park and is Park Board sanctioned. Centerville Park Maintenance Review: May 2021 page 3 of 3 La Motte Park • Field #1 pitcher's mound needs grass seeding • The grass on the east side of the Skate Park needs seeding • Field #2 ump equipment box needs prepping and painting • Scoreboard needs to be replaced. (I picked up the glass that I could see from the light that was broken.) • The metal gridded tops of the picnic tables in the pavilion are warping • The BBQ needs to be cleaned out, also needs to be painted • Rubber chips under the swings and playground equipment need to be refreshed • The new rain barrel needs a hose extension added to be able to water the plants around the park sign • The maintenance garage two small doors need painting • All 4 warming house doors need painting • The wooden boards surrounding the Hockey Rink need painting Recommendations: • It would be a good idea to add a sand (or cement?) bib to the BBQ near the pavilion. • 1 would like the Park Board to consider replacing the balls at the Gaga pit. • If the branch and grass but on the south border of LaMotte is within the park boundaries, for liability reasons I would suggest it be dismantled / removed. • Remove dogwood trees from ditch area near sledding hill to avoid conflict.. Goals • Provide a bulk rate discount to residents who are interested in protecting their trees. • Map private ash trees that received injections to aid in City EAB planning/data gathering. • Educate residents about the benefits of treating ash trees to protect against emerald ash borer. Instructions Proposals must include: 1. Company details and qualifications of all staff overseeing work 2. Fee schedules for private trees (tables are provided on pgs. 5-6) 3. The City reserves the right to negotiate specific work elements with a Proposer into a contract of lesser or greater expense than described in this RFP or Proposer's proposal. The City of Centerville's goal is to offer a preferred vendor to our residents to provide education and reduced rates for insecticide injections for treatment of those trees that will remain viable to continue growth. With increasing levels of infestation apparent throughout our community, it has become critical that the City assist residents with this issue in fashion possible to stream -line the process for residents seeking injections to protect their privately -owned ash trees and offering them education regarding possible infestation and ramifications. The City currently has no program in place for providing treatments for trees on private property. PROPOSER INFORMATION Company Details A proposer must be a properly insured, respected, financially viable company that has been in business for at least five (5) years, with at least three (3) years of experience performing plant health care or injection treatments for local units of government or other public agencies. There must be at least one ISA certified arborist available to supervise the program and the application crew(s). All members of the application crew must be licensed commercial pesticide applicators. Each proposer must provide a list of three (3) municipal or public agency references. Tree inspections Tree inspections shall be performed prior to recommending treatment to residents. These inspections must be performed by a person who is at minimum, a Certified MN Tree Inspector or an ISA Certified Arborist. Individuals recommending treatment must be qualified to identify emerald ash borer in the field and have experience evaluating tree condition, particularly as it relates to determining which trees are suitable candidates for injection and which are not. Treatment of trees exhibiting the following conditions must be noted and reported to the City upon inspection, treatment or none treatment: • More than 30% canopy decline • Signs of severe decay in trunk or major branches • Major cracks in trunk or major limbs • Fungal fruiting bodies present on trunk INJECTION DETAILS Chemical • Injections must be with a liquid formulation of 4% emamectin benzoate • All label directions for chemical product used must be followed. • At a minimum, trees less than 22 inches DBH shall be treated with 2.5 ml of 4% emamectin benzoate solution/inch DBH, trees greater than 22 inches DBH shall be treated with 5 ml of 4% emamectin benzoate solution/inch DBH. Residents' trees Injections performed on residents' privately owned trees will be driven by public demand, so numbers of treatments for these trees will not be available at this time. Residents shall undertake treatments with you if desired but will solely be for their privately owned trees. Private Property The homeowner will contact the Contractor directly to initiate an inspection, receive a quote, and provide permission. The Contractor shall inspect the ash trees on private property and determine whether they are appropriate for treatment. GuarantPP Two-year warranty required for all private injections. If a tree is killed by EAB during this two-year period, or if canopy loss attributable to EAB becomes equal to or greater than 50% during this same period with active beetles or larvae found, a 100% refund will be required. Cost Proposal Proposers shall fill out the included cost and rate forms for private tree treatments. Proposed fee schedule shall include the included tables as well as any additional fees, costs for consultation/assessment visits with homeowners, and taxes. Listing Contractors will provide a listing of all privately owned tree(s) treated by them to the City on a monthly basis. Education and Outreach Materials Contractor shall provide outreach materials, such as a postcard or pamphlet to all residents to inform them about the program. The outreach materials shall be sent in the mail. Contractors shall include both costs for this mailing and examples of previous outreach materials used for similar programs with proposal. The City will supplement the mailed outreach materials with newsletter articles and potentially with posts on City social media accounts. Any other distribution of materials, door knocking, or any other forms of solicitation shall not occur with prior approval from the City. Contract Period Contract shall be in effect for 2022-2023. The contract may be extended upon the City's discretion. Termination The City reserves the right to terminate this contract immediately without cause at any time during the contract. In the event of such termination, the City shall provide Contractor written notice of the termination and upon receipt of same, Contractor shall immediately cease and desist Contractor's provision of services under this contract.
