Centerville Lions Haunted House

This is the list of items that will need to be addressed prior to the Haunted
House event.
1. An evacuation plan will have to be developed and all staff will need to
have a copy of this plan and a working knowledge of the procedures
in it. All patrons and staffwill have to be accounted for.
2. Flood lights will have to be installed throughout the event area and
exits. These lights need to be controlled from a constantly attended
location and turned on immediately if a situation arises.
3. All event sounds and music will need to be shut down with a single
switch.
4. An electrical inspection of all circuits, outlets and fixtures will need to
be completed by a licensed electrician. A signed certificate will need
to submitted stating system is compliant.
5. Occupant load will have to be maintained below 20 participants and
plan for accountability demonstrated.
6. All exit and emergency lights will need to be operational.
7. Fire watch will need to be present. Person must make rounds every 15
minute's. Must have a radio or phone to activate 911. This will be the
only task for this person.
8. Building will only be used for this event on October 19,20,26 and 27
2007 from 1800 hours to 2200 hours.( 600 PM to 1000 PM)
9. Post notice of mold issue at entrance.
10.Insurance binder will need to have City of Centerville and Centennial
Fire District as additionally insured.
l1.In the event this building will be used next year, all applicable City
and State Codes will need to be adhered to.
All the above items need to be completed three days prior to the opening of
the event. October 16, 2007.
As the event is operating spot checks will be performed by the Centennial
Fire District and/or The City of Centerville.

2000 International Fire Code
DEFINITION
202. Special Amusement Building. A building that is temporary,
permanent or mobile that contains a device or system that conveys
passengers or provides a walkway along, around or over a course in any
direction as a form of amusement arranged so that the egress path is not
readily apparent due to visual or audio distractions or an intentionally
confounded egress path, or is not readily available because of a mode of
conveyance through the building or structure.
PERMITS
105.6 Required operational permits. The code official is authorized to
issue operational permits for the operations set forth in sections 105.6.1
through 105.6.46.
105.6.2 Amusement buildings. An operational permit is required to operate
a special amusement building.
ALARMS
907.2.11 Special amusement buildings. An approved automatic smoke
detection system shall be provided in special amusement buildings in
accordance with this section.
Exception: In areas where ambient conditions will cause a smoke
detection system to alarm, an approved alternative type of automatic
detector shall be installed.
907.2.11.1 Alarm. Activation of any single smoke detector, the automatic
sprinkler system or any other automatic fire detection device shall
immediately sound an alarm at the building at a constantly attended location
from which emergency action can be initiated, including the capability of
manual initiation of requirements in Section 907.2.11.2.
907.2.11.22 System response. The activation of two or more smoke
detectors, a single smoke detector with alarm verification, the alitomatic
sprinkler system or other approved fire detection device shall automaticlly:
1. Cause illumination of the means of egress with light of not less than 1
foot-candle (11 Lux) at the walking surface level;
2. Stop any conflicting or confusing sounds and visual distractions; and
3. Activate an approved directional exit marking that will become
apparent in a emergency.
Such system response shall also include activation of a prerecorded
message, clearly audible throughout the special amusement building,
instructing patrons to proceed to the nearest exit. Alarm signals used in
conjunction with the prerecorded message shall produce a sound which is
distinctive from other sounds used during normal operation.
The wiring of the auxiliary devices and equipment used to accomplish
the above fIfe safety functions shall be monitored for integrity in
accordance with NFP A 72.
907.2.11.3 Emergency voice/alarm communication system. An
emergency voice/alarm communication system, which is allowed to serve as
a public address system. Shall be installed in accordance with NFP A 72 and
be audible throughout the entire special amusement building.
SUPPRESSION
903.2.15 Other required suppression systems. In addition to the
requirements of Section 903.2, the provisions indicated in Table 903.2.15
also require the installation of a suppression system for certain buildings and
areas.

903.2.15 Other required suppression systems. In addition to the
requirements of Section 903.2, the provisions indicated in Tanble 903.2.15
also requires the installation of a suppression system for certain buildings
and areas.

411.1 General. Special amusement buildings having an occupant load of 50
or more shall comply with the requirements for the appropriate Group A
occupancy and this section. Amusement buildings having an occupant load
of less than 50 shall comply with the requirements for the appropriate Group
B occupancy and this section.
Exception: Amusement buildings or portions thereof that are without
walls or a roof and constructed to prevent the accumulation of smoke.
For flammable decorative materials, see the International Fire Code.
411.2 Special amusement building. A special amusement building is any
temporary or permanent or portion thereof that is occupied for amusement,
entertainment or educational purposes and that contain a device or system
that conveys passengers or provides a walkway along, around or over a
course in any direction so arranged that the means of egress path is not
readily apparent due to visual or audio distractions or is intentionally
confounded or is not readily available because of the nature of the attraction
or mode of conveyance through the building or structure.
411.3 Automatic fire detection. Special amusement buildings shall be
equipped with an automatic fire detection system in accordance with Section
907.
411.4 Automatic sprinklers. Special amusement buildings shall be
equipped throughout with an automatic sprinkler system in accordance with
Section 903.3.1.1. Where the special amusement building is temporary, the
sprinkler water supply shall be of an approved temporary means.
Exception: Automatic fire sprinkles are not required where the total
floor area of a temporary special amusement building is less than
1,000 square feet and the travel distance from any point to an exit is
less than 50 feet.
411.5 Alarm.. Activation of any single smoke detector, the automatic
sprinkler system or any other automatic fire detection device shall
immediately sound an alarm at the building at a constantly attended location
from which emergency action can be initiated, including the capability of
manual initiation of requirements in Section 907.2.11.2.
411.6 Emergency voice/alarm communication system. An emergency
voice/alarm communications system shall be provided in accordance with
Sections 907.2.11 and 907.2.12.2, which is also permitted to serve as a
public address system and shall be audible throughout the entire special
amusement building.

411.7 Exit marking. Exit signs shall be installed at amusement building
required exit and exit access doorways. Approved directional exit markings
shall also be provided. Where mirrors, mazes or other designs are utilized
that disguise the path of egress travel such that they are not apparent,
approved low-level exit signs and directional path markings shall be
provided and located not more than 8 inches above the walking surface and
on or near the path of egress travel. Such markings shall become visible in
an emergency. The directional exit marking shall be activated by the
automatic detection system and the automatic sprinkler system in accordance
with Section 907.2.11.2
411.8 Interior rmish. The interior finish shall be Class A in accordance with
Section 803.1
803.1 Interior finishes: General. Interior wall and ceiling finishes shall be
classified in accordance with ASTM E 84. Such interior finish materials
shall be grouped in the following classes in accordance with their flame
spread and smoke-developed index.
Class A: Flame spread 0-25; smoke developed 0-450
Class B: Flame spread 26-75; smoke developed 0-450
Class C: Flame spread 76-200; smoke developed 0-450
Fire Watch: A temporary measure intended to ensure continuous and
systematic surveillance of a building or portion thereof by one or more
qualified individuals for the purposes of identifying and controlling fire
hazards, detecting early signs of unwanted fire, raising an alarm of fire and
notifying the fire department.

Re: Hidden Spring Park construction issues to be addressed

To Honorable Mayor and City Council,
I want to thank Mayor Capra, Suzanne Seeley, and Dallas Larson for meeting Dale Tranter and me at
Hidden Spring Park to discuss your concerns. I genuinely appreciate your candid comments and willingness
to work with us on this project. Your commitment to rectifying this issue openly, honestly, and
professionally is appreciated.
We have met with Veit Construction to discuss solutions to your concerns. Below you will find an outline
of each item we discussed at our meeting, a solution to the item, and a timeline for getting this work done.
We have asked Veit to co-sign this letter to show our mutual commitment to following through on these
items.
1. Graffiti in the concrete sidewalk on the LaMotte Park side of LaMotte Drive.
a. Remedy - The concrete will be replaced and backfilled.
b. Timeline - COMPLETED
2. The rainwater garden overflow is in the middle of the raingarden instead of at the designed
overflow location.
a. Remedy - The overflow will be incorporating the recommendations of Bonestroo's water
resources engineer. See the attached memo.
b. Timeline - By October 1 th
3. Draintile was added to the raingarden. This newly added draintile does not discharge at the head
of the swale. The City is wondering if the discharge point decreases water quality treatment.
a. Rice Creek Watershed District has reviewed the draintile. They have written a letter saying
the draintile as installed is acceptable. See the attached memo from Elizabeth Muller of
Rice Creek Watershed District.
4. The outlet for the added draintile is unsightly.
a. Remedy - Stone will be added to the outlet of the draintile.
b. Timeline - By October 12th

5. The City wondered if the hydrant could be used to test how long it takes for the raingarden to
empty.
a. Following the September 10th meeting when this issue was brought up, the area received.
substantial rain. Jeff McDowell visited the raingarden approximately 24 hours after the
rain event. Wood chips along the side of the raingarden clearly showed the raingarden
had filled to capacity. The water level had dropped approximately 12 inches within those
24 hours, and the draintile was flowing slow and steady. This information was forwarded
to Gregg Thompson, and he responded by saying the drain-down times seemed good to
him (see attached email chain).
b. If the City would like a more controlled drain-down test, Bonestroo will use the hydrant to
fill the raingarden and time how long it takes to empty.
6. Can the raingarden sand/compost mix be tested to show compliance with the design?
a. Remedy - Gregg Thompson visited the site and used a penetrometer to test the
raingarden soils. He was able to easily push in the penetrometer to its full depth of 30".
Gregg responded that the soils" appear to be in really good shape". In addition, Veit has
submitted the raingarden soil mix tickets from Plaistads. See attached email
correspondence and tickets.
b. Timeline - COMPLETED
7. Can we verify that the raingarden is meeting the design requirements?
a. Remedy - An as-built survey was conducted to verify that the built raingarden meets the
Watershed District's requirements. See the attached memo from Earth Evans.

b. Timeline - COMPLETED
8. Plants in the raingarden and along the side of the raingarden have died.
a. Remedy - A replacement plant list will be provided, and the dead plants will be replaced.
b. Timeline - By October 12, 2007
9. Add a gravel path near the shoreline to clearly define pedestrian walkways through the plantings.
a. Remedy - At the direction of the Council, three gravel extensions will be built.
b. Timeline - By October 12, 2007
10. The biorolls were installed using two methods. One method has the bioroll pinned to the shoreline
at the water level. The second has the bioroll offset from the shoreline. The City has asked which
installation method is correct.
a. The intended method of construction is to have the biorolls away from the shore, and
acting as a wave break. The design also calls for aquatic plants to be planted between
the bioroll and the shoreline. Ultimately, it's the roots from the aquatic plants that provide
erosion protection. The bioroll is meant to be a temporary measure to protect the plants
during establishment.
b. Remedy - Bonestroo will provide additional shoreline stabilization in the spring of 2008.
Per Gregg Thompson's email, Bonestroo will plant additional shrub and perennial
plantings along the shoreline. In addition, Bonestroo will provide a temporary wave break
during plant establishment. We are currently evaluating wave break options, and will
work with the Watershed District and the City on an acceptable solution.
c. Timeline - Spring of 2008. Because the growing season has come to an end, it's best to
wait until next spring to install the plants and temporary wave break.

11. The farthest north section of shoreline does not have a bioroll.
a. Remedy - Biorolls will be installed, topsoil backfill will be placed along with erosion
control blanket and replacement plantings.
b. Timeline - October 12, 2007
12. The floating silt fence has floated into shore. In some areas it has jumped the bioroll stakes and
has washed up next to the plants.
a. Remedy - Bonestroo will seek the Watershed District's approval to remove the floating
silt curtain. Once approval is granted, the floating silt fence will be removed.
b. Timeline - Fall of 2007
13. A portion of the gravel trail near the lake eroded.
a. Remedy - A row of sod (approximately 3' x 20') will be added along the edge of the trail,
and the trail will be regraded.
b, Timeline - In Progress
14. The rip rap near the lake is eroding around the edges.
a. Remedy - Fabric along the edge of the channel will be provided and rip rap replaced.
b. Timeline - October 12, 2007
15. Near the swing there is a depression in the path.
a, Remedy - The depression will be filled with gravel.
b. Timeline - October 12, 2007
16. The rock outcroppings at the raingarden have voids.
a. Remedy - Voids will be filled with concrete.
b. Timeline - October 12, 2007
17. At the head of the artesian well, the rocks should be large enough to be difficult for people to
move.
a. Remedy - A rock that is approximately 3' x 4' x 6" (or two rocks that are 12") will be
provided and placed over the artesian well.

18. A tree that was trimmed did not get dressed.
a. Remedy - Wounds will be dressed.
b. Timeline - COMPLETED
19. The bike rack landing area was sodded when it should be gravel.
a. Remedy - Sod will be removed.

20. Park bench support poles are not at the same elevation.
a. Remedy - Benches will be leveled.

21. Drinking fountain is not plumb.
a. Remedy - Fountain will be plumbed.

Re: Rice Creek Watershed District Permit #07-059, Hidden Springs Park, Centerville
The RCWD conducted a site inspection at the project identified above on September 10,
2007. This letter serves to follow up on the tile drain discussions held on site at the time
of inspection.
The additional drain tile is acceptable in terms of the Rice Creek Watershed District
permit. The necessary treatment for this site is contained within the main part of the
raingarden. The swale exists to convey and treat overflow from this main raingarden in
large rainfall events.
The swale does need to be operating as the overflow path rather than the overland area
where the drain tile was installed. This was discussed during the site meeting and the
necessary adjustments were agreed upon.
These items are part of RCWD standards and are enforceable under RCWD Rule (MS
103D). If you have any questions, please contact me at 763-398-3074.

I visited the raingarden on Wednesday approximately 24 hours after Tuesday's rain. Floating woodchips clearly
showed that the raingarden had filled to it's capacity. Within the 24-hours since the rain, the water level had
dropped approximately 15 inches. There was still about 10 inches of water in the raingarden. The draintile was
flowing slow and steady.
Based on this site visit, it seems to me that the raingarden is meeting the retention needs.

Gregg,
Thanks a lot for the evaluation. The council and parks folks are going to meet on site at 6:00 on Wednesday the
26th. We would love to have you there if you could make it. If you want to bring your power point, we could
view it at the council meeting afterwards (7:00).
A couple of things I could clarify for you:
. Side slopes of the rain garden were only recently planted and haven't had much of a chance to fill in. We
do, however, plan to add some more plantings to areas without plants.
. The rain garden has been draining in sufficient time since we modified the drain tile outlet. Originally, the
draintile ran into the outlet structure. The invert of the outlet structure, however, was higher than the
draintile violating the old rule that water does not run uphill. Since we bypassed the outlet structure and
took the draintile into the swale to outlet, the garden has drained properly. It was during this time that the
rain garden drain tile was not functioning, that most of our plants died. We plan to replace those plants
(some in kind because we think they will survive now that the rain garden is functioning properly and others
we will replace with more hearty species such as those which have made it despite the prolonged
inundation).
. The floating silt curtain has never been meant to be a wave breaker and has caused us a lot of headaches.
It's there as part of our temporary erosion control plan. We will be removing it soon.
Thanks again. We will run some of your suggestions past the council and parks board to see if they would like to
install those items.

Bioretention Area:
Upon our site visit, the surface of the bioretention / raingarden area was dry. We used a soil-penetrometer on the replacement
soils in the raingarden and we were easily able to push in the penetrometer to its full length of 30" - soils in the raingarden appear
to be in really good shape, although we didn't have a device deep enough to hit the sub-soils area.
How long has the raingarden been ponding water after it has been filled up to the overflow elevation? Is the under-drain
functioning as intended?
Vegetation in the basin & side slopes has some gaps / voids (some species didn't make it) and should be filled in with plants that
have proven to establish successfully in these situations (such as the shrubs, sedges, iris, etc.).
Shoreland Stabilization / Buffer:
Two biofogs appear to have come loose from the bank toe at the north end of the shoreline project. They should be re-staked /
anchored.
Some planting areas in the shoreland transitional zone have a an uneven distribution of sedges and other strongly-rooting plants.
Because of the wave-pressures that will always exist, the transitional area should be planted more densely with more sedges and
grasses, among the existing shoreland plantings. The biologs should also be planted (at a 12" spacing) with aquatic/transitional
species, such as three-square bulrush, lake sedge, etc.
Also, regarding the wave-breaker / floating silt-curtain, it may be a good idea to install something, like a cedar tree revetment along
the shoreline in front of the bi%gs in the areas most heavily impacted by wave action / slope degree. The Anoka Conservation
District has recently harvested some cedar trees off of a site that was being bulldozed in Blaine. We could transport down some of
these cedar trees, but anchors would need to be purchased to secure them in place.
I'm attaching a powerpoint fiie with photos from my site visit on 9/14/2007, along with notes on the slides of my observations and
suggestions. Overall, I think the project looks nice and seems to be working well - I would just want to hear more on how long the
raingarden area has standing water in it after the rain has stopped.

Rain Garden Stormwater Analysis

Remarks:
This memo summarizes our stormwater analysis of the Hidden Spring Park project. A chain of
treatment measures were constructed for the project including a small rain garden, swale,
depression storage and shoreland protection. Post-construction issues with slow drawdown in
the rain garden and draintile re-routing led to additional analysis of the stormwater system.
Rain Garden Sizing
Runoff from the LaMotte Drive storm sewer system discharges into a small rock fore bay then
the rain garden. The rain garden functions as the primary water quality treatment feature. It
was sized based on Rice Creek Watershed District (RCWD) proposed new rules that require
infiltration/biofiltration volume adequate to contain the 0.8-inch event (which exceeds their
existing requirement of the runoff from the 0.34-inch event). The rain garden was surveyed to
determine what volume is actually available based on constructed elevations. The following
table compares the as-built volume and RCWD requirement.

As shown in the table, the volume provided in the rain garden is adequate to meet the RCWD
proposed rule if the 893 contour is closed around the entirety of the rain garden. The minimum
elevation for the outlet structure is 892.9 to still provide adequate volume to meet the
proposed rule. Technically RCWD cannot require conformance to the new rules until they are
adopted (likely December 2007), but the site was reviewed based on compliance with the new
rules.
Overflow Elevation
The rain garden was designed with three outlets - draintile, 12-inch outlet structure and
overflow to the swale. Due to the clayey soils, draintile was utilized to allow for complete
drawdown of the rain garden. The 12-inch outlet structure is the primary outlet and along with
the grass overflow is routed to the downstream swale. The purpose of this design is to:

· restrict the discharge for larger storm events to the overflow location and
therefore minimize erosion
As the rain garden is currently constructed the overflow is southwest of the swale through the
center of the park. The ground elevation should be raised to force the overflow to the swale. If
the existing outlet structure remains as 893.06 elevation, the overflow elevation should be set
at 893.6 to contain the 5-year and smaller events in the rain garden. Therefore the ground
elevation everywhere else in the rain garden should be approximately 894 and above in order
to force the overflow to the swale. Extreme events would not stay localized in the swale, but
the majority of storm events would be contained in the rain garden and swale system.
If the primary outlet elevation is lowered to 892.9 (the minimum elevation to meet the RCWD
proposed rules), then the minimum overflow elevation is 893.5.
Draintile
As noted above, draintile was provided due to the hydrologic soil group D soils. The rain
garden functions more as a biofiltration area then an infiltration area. This method is also
allowed in the RCWD Rules. Filtration of larger particles in the engineered soils at the base of
the rain garden, uptake in the root structure of the plants and evapotranspiration are the
primary means of water quality treatment. There is nominal volume infiltrated, but it was not
considered since the soils are dominated by clays.
The as-built survey shows that a minimum of one foot of separation is provided between the
bottom of the rain garden and the draintile invert. This sand layer acts as a filter system. The
compost in this layer allows for growth of the plants. The plants then remove phosphorus,
nitrogen and other compounds from the stormwater runoff.
The draintile was re-routed to the lower depression, bypassing the swale. The original intent of
the design was to route the draintile to the swale. Although the water quality treatment
achieved in the swale was never directly accounted for since it is insignificant. Therefore
routing the draintile to the swale first would not significantly add to the water quality
treatment. A longer, very gradual sloping, vegetated swale functions better for water quality
purposes, but conversely can lead to pooling and stagnant water if not graded correctly.
