(SEHP) is pleased to submit this letter proposal for inspecting and <br />assessing the condition of the. City's sanitary. sewer manholes (Project). This letter supplements the <br />Agreement for Professional Services between the City.of Gem Lake (Owner) and SEH (Consultant) for <br />providing professional services. <br />SCOPE~ OMORK <br />The purpose of this work.scope is to complete the inspection and condition assessment of the <br />( approximate. 65 sanitary sewer manholes located within the City of Gem LaWs sanitary sewer system. <br />See Attachment 1 for SEH's understanding of the number and locations of the City's existing sanitary <br />sewer manholes. Manhole numbers 6, 28, 40'and 50 will be investigated to confirm if they are now <br />accessible and if so, will be inspected. <br />All of the manholes will be scanned using the Rapid View PAN0RAM0.S1 camera from IBAK.. The <br />camera captures images which can. be used to complete a. 360-degree perspective view of each <br />manhole, which can be used at any time from any angle. A database will be created by SEH based .on the <br />standards identified under the National Association of Sewer Service Companies (NASSCO) and criteria <br />established under its Manhole Assessment Certification Program (MACP) in order to assess each <br />manhole condition. SEH will utilize a modified MACP Level.2 inspection format In order to best fit the <br />City's needs. All camera operators, inspectors and SEH .staff coding off of manhole scans have a current <br />PACPIMACP Certification, All inspectors are corifined space certified. <br />Projecf Kickoff Meeting <br />SEH will review access requirements for each structure to be inspected, the anticipated project schedule: <br />and the list of attributes to be collected and/or coded as required under NASSCO's MACP Level 2 <br />requirements. The following Information will be provided by the City of Gem Lake (if available): <br />1. All existingsewer collection system data in an ArCGIS .geodatabase format for use during the <br />project, Including .any current GIS locations. <br />2.
