Robert Uzpen Mayor City of Gem Lake 4200 Otter Lake Road Gem Lake, MN 55110�-5231 Dear Mayor Uzpen: RE: City of Gem Lake, Minnesota Scheuneman Road Reconstruction SEH No. GEMLK 143869 14.00 We. are pleased to submit this letter proposal for providing engineering services relating to the construction of the Scheuneman Road Reconstruction and Water Main Extension Project. This project'. Was bid on April 19, 2018. and construction is anticipated to commence in the spring of 2018 with a final. completion date of September 15, 2018. This letter. supplements the Agreement for Professional Services between the City of Gem Lake, Minnesota (Owner), and Short Elliott Hendrickson Inc. (SEHO) (Consultant), for providing professional services. SCOPE OF WORK 1. Provide construction administration and construction staking as needed during the construction of the improvements. 2, Provide daily on -site. construction observation services during the construction of the improvements. The construction schedule is currently estimated to take approximately 5 weeks, We have estimated the on -site observation time at 200 hours. The on -site observation will be billed on an, hourly basis and may be impacted by forces beyond our control, such as.the weather, contractor's schedule and how efficiently he prosecutes his work: We will provide the City updates on the project's progress and how the observation budget estimate is tracking.. 3. Coordinate material testing services with an outside consultant, The: City will be invoiced for these services by the testing .consultant directly.. 4. Provide construction administration during the project including coordination of preoonstruction meeting; review contractor applications for payment and communications with the City during. construction: 5. Prepare "as -built', construction drawings of the improvements in. hard copy and electronic AutoCAD format: SCHEDULE As stated above construction is anticipated to begin in June of 2018 with final completion. by September 15, 2a18. A pre -construction conference will be held prior to construction with the City and Contractor, as well as private utility representatives, If there are delays in the Project that are beyond our control, the City agrees to grant additional time and compensation to complete. the services.
