or unauthorized changes to the system design that alter the original function of the system. as <br />detemnined. by the Department; change the treatment capacity of the system, change the location <br />of the system, or otherwise .change the original system's design; layout, or function. A notice of <br />suspension or revocation and. the reasons for the suspension or revocation shall be conveyed in <br />writing to the permit holder. If suspended or revoked, installation or. modification of a treatment <br />system may not commence or continue until a valid. Construction.Permit is obtained. <br />(k) Posting <br />The Construction Permit shall be posted on the property in such a location and manner so <br />that the permit: is visible and available for inspection until Construction is completed and certified: <br />Sec. 23 - OPERATING PERMIT <br />(a) SSTS Requiring an Operating. Permit <br />An operating Permit shall be required of all owners of new holding. tanks, Type IV <br />Systems, Type V 'Systems, MSTS, or any other. system deemed by the Department to require. <br />operational oversight. Sewage shall not. be discharged to these systems until the Department <br />.certifies that the system was installed in substantial conformance with the approved plans, receives <br />the final record drawings` of the system, and a valid Operating Permit is issued to the. owner. <br />(b) Permit Application Requirements <br />(1) Application for an Operating Permit shall be :made on a form provided by the <br />Department including: <br />a. Owner name, mailing address, telephone, and email address <br />b. Construction Permit reference number and date of issue. <br />c. Final record drawings of the treatment system <br />
