Proposals must include <br /> the following information: <br /> 6.1 Introductory letter explaining consultant understanding of the request. <br /> 6.2 identification of consulting team to perform the tasks in the scope of work. <br /> The name address, phone, and fax number of a contact person should <br /> be listed. <br /> 6.3 Consulting team experience and qualifications. <br /> 6.4 List of comparable studies completed or in progress. <br /> 6.5 Schedule for completion of study. <br /> 6.6 cost breakdown for each portion of the proposal <br /> Dote: The ro osal should not include reproduction casts and mapping <br /> p p <br /> costs.
