CITY OF ROSEVILLE <br /> FLEXIBLE BENEFITS <br /> REGULAR PART-TIME EMPLOYEES <br /> (NON-UNION EMPLOYEES ONLY) <br /> NOTE: The following benefits represent a city contribution of one-half of what <br /> is being offered to regular full-time employees under the flexible benefit plan <br /> and wi11 remain at the one-half level in succeeding years. <br /> EMPLOYEES CHOOSE ONE OF THE FOLLOWING OPTIONS: <br /> do <br /> Option A: Employee chooses a City contribution of $145 toward <br /> the health insurance premium for employees choosing <br /> family coverage. For employees choosing single <br /> coverage, the City contributes one-half of the single <br /> coverage health insurance premium rate. <br /> Option B: Employee chooses a monthly city contribution toward <br /> deferred compensation of $65/month, <br /> OR <br /> Option C: Employee chooses a monthly cash benefit of <br /> $ <br /> F OR <br /> Option D: Employee chooses 40 hours of vacation per year. <br /> Vacation must be taken during the plan (calendar) year <br /> or it will be forfeited. <br /> III <br />
