<br />private residences., dwellings and apartment houses
<br />located within two hundred (20101) feet of such prern-
<br />ises to the effect that they have no' objection to
<br />the granting of'such license or the operation of
<br />the Private club at the location proposed,- provid-
<br />ed. nevertheless,,, that no consents shall bile neces-
<br />sary when the premises whereon the "
<br />private club
<br />is proposed, i,s, zoned as 1-1,, 1-2, or 1-,3 Inidu,s�tria,l
<br />District. Private clubs already licensed on, the
<br />effective date of this section, (June 6. 1,9,771 may
<br />continue to operate and have their licenses re-
<br />newed, without regard, to the requirements of' this
<br />section, sio, long as such licensees continue to coim-
<br />ply, with all other provisions of this chapter and
<br />other laws. Applications, for new licenses and fair
<br />the transfer of licenses as, defined in Section 3101.11
<br />made after the, elffectiive date, of this section shall
<br />,comply with the consent requirements, contained
<br />in this section.,

<br />,Sec., 40 06. Application,
<br />(a) Anv person desiring a license under this
<br />chapter shall make application in writing to the
<br />inspector on such forms, containing such informa-
<br />ti,on as he may require, which shall at a mini-
<br />mum contain the following:
<br />(1) The name and place. of'. residence of' the
<br />applicant;
<br />(2), The exact location of the, premises upon
<br />which, the applicant proposes to, operate the
<br />private club-,

<br />) Whether the applicant, has ever been en-
<br />gaged, in a, similar business and., if so, the
<br />location thereof and the date when so
<br />engaged:
<br />14,) Whether the applicant proposes, in ad,di-
<br />tion to the, c( -iiductmig of meetings,, to provide:
<br />a. Instrumental music with dancing for
<br />the guests therein-,
<br />bi. For the serving of food for consump-
<br />tion on the premlsesw,
<br />c. Nonintoxicating beverages for consump-
<br />tion on the premises, and
<br />complete listing of the names, addresses
<br />and telephone numbers of each member of
<br />the private club, together with the date
<br />each such member joined such club.
