<br />We continue to recei1ve Requests for Inspecti'lon with fees, which include the Sltate
<br />surcharge,., The State surcharge applies I to municipal insplection departments.,
<br />Please do, not include a surcharge with fees for areas under the Board of Electricity's
<br />inspection.

<br />write any instructions or maps for t,he inspectorl on the black of the whitei copy.1

<br />An electrical contractor's or alarm and communication ciontiractior'.si license is valid
<br />for one business only. It is unlawful to file a, Request for, Electrical Inspection
<br />I done by- anyone other than the contractor. Anyonel performing the, work for
<br />th�e contractor must be an emplaee., with, his compensation for, the work reported on
<br />a W-2 form. Persons who have theilr compensation repoirit,ed on a 1099 form a,rei not
<br />emplolyees,, and c,annot do electrical work.
