Appearance of an altered identification document <br />• Inconsistencies between submitted information and information on file and/or other <br />document sources <br />Employee Procedures <br />Any employee that suspects fraud or detects a red flag is expected to (where applicable): <br />• Ask the applicant for additional documentation to verify authenticity. Absent <br />verification, the employee shall: <br />1) Refrain from creating/modifying an account, and <br />2) Notify immediate supervisor <br />• If applicable, the Supervisor shall notify the Roseville Police Department of any <br />suspected fraud. <br />In an effort to aid the detection of fraud and prevent identify theft, the City of Roseville adopts <br />the security procedures outlined in Appendix A. <br />Identity Theft Prevention Prog,r m Review and Approval <br />This plan has been reviewed and adopted by the Roseville City Council by Resolution # <br />dated November 10, 2008. Appropriate employees have been trained on the contents and <br />procedures of this Identity Theft Prevention Program. <br />This plan shall be reviewed periodically to ensure the effectiveness of the policies and <br />procedures contained therein. If applicable, a report shall be presented to the City Council <br />regarding any identify theft incident, any response to the incident, and any recommendations for <br />changes to the program necessary to prevent future occurrences. <br />0 <br />
