AGREEMENT FOR <br />EMERGENCY MEDICAL TECHNICIAN <br />SERVICES <br />THIS AGREEMENT, is entered into this _ day of August 2004, by and between the <br />City of Roseville, a political subdivision ("City") and the Roseville Area Youth Football <br />Association, anon-profit corporation ("RAYFA") <br />WHERAS, the City Fire Department has certified Emergency Medical Technicians <br />("EMT") on staff; <br />WHEREAS, R.AYFA acknowledges that the City does not provide paramedic, <br />ambulance, or medical transportation and understands that those services are not part of <br />this Agreement; <br />WHEREAS, RAYFA and the City desire to enter into this Agreement for the purpose of <br />providing EMT coverage for RAYFA youth football games at an agreed price. <br />NOW THEREFORE, IN CONSIDERATION OF the foregoing and the mutual <br />promises and agreements contained herein, the .parties agree as follows: <br />1. Responsibilities of the City. <br />(a) Provide stand-by EMT services at RAYFA youth football games. <br />(b) Provided medical supplies consistent with an EMT level of medical care. <br />(c) Issue invoice no later than November 15 for the EMT hours of service that <br />are provided under this Agreement. <br />2. Responsibilities of RAYFA. <br />(a) Provide City a list of dates and locations of R.AYFA youth football games <br />no later than 14 days before the first game. <br />(b) Maintain a log of the medical and/or allergic conditions that RA~.'FA <br />knows its participants have and have that log available at all RAYFA youth <br />football games. <br />3. Payment. RAYFA shall pay the City at a rate of $21.00 per hour for the EMT <br />services. Payment shall be made in one lump sum payable no later than <br />November 30. <br />4. Term. The term of this Agreement shall be from the date first stated above <br />until October 31, 2004. This Agreement shall be automatically extended from <br />EMT AGREEMENT <br />Page 1 of 2 <br />
