1 <br /> <br />Agreement for Curbside Collection of Appliances Services <br /> <br /> <br />This Agreement (“Agreement”) is made on the ______ day of _________________, 20 _____, <br />between the City of Roseville, a municipal corporation (hereinafter “City”), and Better Futures <br />Enterprises , a Minnesota Corporation, with its principal place of business at 1017 Olson <br />Memorial Highway , Minneapolis, Minnesota 55405 (hereinafter “Con tractor”). <br /> <br />Preliminary Statement <br /> <br />The C ity has adopted a policy regarding the selection and hiring of contractor s to provide a <br />variety of professional services for City projects. That policy requires that persons, firms or <br />corporations providing such services enter into written agreements with the City. The purpose of <br />this Agreement is to set forth the terms and conditions for the performance of professional <br />services by the Contractor. <br /> <br />The City and Contractor agree as follows: <br /> <br />1. Scope of Work Proposal. The Contractor agrees to collect at curbside, appliances from <br />residents participating in the pilot program, and transport them to the Contractor’s facility <br />for processing. The Contractor will generate information on the number of appliances <br />collected, type of appliances collected and the disposition of the appliances (e.g. recycled, <br />refurbished). The Contractor will report that information to the City no later than June 30, <br />2013. There will be curbside collections the first week of April, May and June 2013. A <br />list of the type of ap pliances to be collected is Attachment A to this contract . <br /> <br />The City will identify the households to be included in the pilot program, and the City <br />will write, design, print and mail communications regarding the pilot program to these <br />households. The City will receive phone and email communications from residents <br />wishing to participate in the pilot program by registering their appliance(s) for collection <br />and each month will provide an address list to the Contractor no later than Thursday of <br />the week prior t o collection. <br /> <br />2. Term. The term of this Agreement shall be from March 19 , 201 3, through June 30 , 2013 , <br />the date of signature b y the parties notwithstanding. <br /> <br />3. Compensation for Services. The City agrees to pay the Contractor the following <br />compensation: <br /> <br />The City will pay the Contractor $1,800 (billed in three monthly installments of $600) <br />which will provide for collection of up to 120 appliances during the term of the contract. If <br />residents register more than 120 appliances for collection, the City will pay the Contractor <br />$10 for each appliance collected in excess of the original 120 . Final payment will not be <br />made until all responsibilities under Section 1 above are met. <br />
