<br />i had the pleasure of seeing Patricia Sagert in neurosurgical foilow-up today.
<br />She underwent a C5-6 anterior cervical discectamy and fusion on September
<br />12, 2006. This was as the result of a motor vehicle accident, and she had
<br />quite a bit of muscle spasm prior to her accident. Since her surgery she has
<br />continued with difficulties of stiffness and spasm in her neck and shou(ders.
<br />She has continued with physical therapy. She has a fot of tingling in her feet.
<br />We tried her on Neurontin and Lyrica in the past, and this did not help. She
<br />notes she was started on Ambien, which helps with sleep, and she is
<br />satisfied with that treatment. She has had increasing headaches with driving
<br />or computer use, and this is most notably related to muscle spasms in her
<br />neck. She has an appointment with an ENT physician tomorrow, as her
<br />voice cames and goes, and they will evaluate her for that.
<br />Sh� continues with chiropractic care once per week and has massage at her
<br />chiropractor's office at that time. She alsa has been receiving physical
<br />therapy once or twice a week and notes she is doing a(ot of stretching
<br />exercises.
<br />We talked about yoga for some of the siretching, and she would like to

<br />�"5,��,���t �;<<,+,+�„� participate in that. However, last week she had a uterine fibroid removed
<br />n'""""` '�`'' `"`" and therefore is limited in what she can do for whole body yoga training.

<br />We discussed the appropriateness of her following with us on a regufar
