I've attached a PDF copy of that application. <br />You'll find that the first 3 pages of the application describe the process (although the <br />timeline illustrated on the first page is more representative of the process for a new PUD <br />rather than an amendment), which includes the requirement that you (or the adult day care <br />provider) hold an open house meeting to describe the process. Typically, applicants submit <br />the 4th page of the application materials to request the mailing addresses for the property <br />owners and others who are to receive invitations to the required open house meeting; since I <br />know that time is an issue for you, I've attached the addresses to this email rather than <br />waiting for you to submit that request form. The last two pages of the PDF application <br />materials are the application form itself. <br />Officially, the submission deadline for applications is Friday, September 3rd, but we can <br />accept your application through September 10th. I understand that the goal is to bring your <br />application for final action by the City Council at its meeting on October 11, 2010. Working <br />backward, we'll plan to schedule the public hearing by the Planning Commission at its special <br />meeting on September 29th; in order to do that, I need submit the public notice to the legal <br />newspaper by September 10th and you will need to submit the summary of the required open <br />house meeting no later September 17th. If we don't receive the open house meeting summary by <br />then, however, the public hearing by the Planning Commission will have to be pushed back <br />until November 3rd (delaying City Council action to at least November 8th) because we will <br />have missed the opportunity to make adequate public notifications for an earlier Planning <br />Commission meeting date.
