I've attached a PDF copy of that application. You'll find that the first 3 pages of the application describe the process (although the timeline illustrated on the first page is more representative of the process for a new PUD rather than an amendment), which includes the requirement that you (or the adult day care provider) hold an open house meeting to describe the process. Typically, applicants submit the 4th page of the application materials to request the mailing addresses for the property owners and others who are to receive invitations to the required open house meeting; since I know that time is an issue for you, I've attached the addresses to this email rather than waiting for you to submit that request form. The last two pages of the PDF application materials are the application form itself. Officially, the submission deadline for applications is Friday, September 3rd, but we can accept your application through September 10th. I understand that the goal is to bring your application for final action by the City Council at its meeting on October 11, 2010. Working backward, we'll plan to schedule the public hearing by the Planning Commission at its special meeting on September 29th; in order to do that, I need submit the public notice to the legal newspaper by September 10th and you will need to submit the summary of the required open house meeting no later September 17th. If we don't receive the open house meeting summary by then, however, the public hearing by the Planning Commission will have to be pushed back until November 3rd (delaying City Council action to at least November 8th) because we will have missed the opportunity to make adequate public notifications for an earlier Planning Commission meeting date. If you don't think you can provide the summary of the open house meeting by September 17th, be sure to let me know by September 10th so that I can submit the public notifications for the Planning Commission meeting on October 6th (which will still keep you on track for the October 11th Council meeting).
