• <br /> 1- <br /> Attachment A - Proposal Content Checklist <br /> Proposers must prepare six hard copies and one electronic copy of the submissions that follow the format and <br /> sequence specified in this RFP. Submission must be printed on 100%post-consumer recycled content paper and <br /> one electronic copy of the proposal formatted in Microsoft WORD on a compact disk that is preferably <br /> reformatted/recycled. <br /> Proposers shall complete and submit this checklist of items for inclusion in the proposal. Each item must be <br /> submitted on a separate page. This checklist may also be filled out electronically using the attached Form <br /> version. <br /> Responsiveness (Pass/Fail) <br /> The Contractor must submit the following in order to be considered responsive: <br /> ❑ Completed Proposal Submittal Form and Affidavit of Non-Collusion (Attachment B) <br /> ❑ Statement as to any litigation in the past five years within the State of Minnesota and the current status of <br /> that litigation <br /> ❑ List of firm's Principal Officers' names, and name, addresses and contact information (telephone, email, fax) <br /> for designated contact person <br /> ❑ A comprehensive cart rollout plan and schedule for the initial cart distribution (Section 5.02) <br /> ❑ A collection plan to coordinate the pickup of used bins, lids and wheel kits from resident's homes with the <br /> delivery of the new carts (Section 5.23) <br /> ❑ Examples of monthly and annual reports (Section 6.01) <br /> ❑ Example of the Contractor's annual report to MFD building owners (Section 6.03) <br /> ❑ A statement clearly specifying the location(s) of its recyclables processing facility (or subcontractor's <br /> facility) where material collected from the City will be delivered and/or processed (Section 7.01) <br /> ❑ Certification of end markets (Section 7.06) <br /> ❑ Examples of proposer's public education materials including education tags (Section 5.07 and 6.05) <br /> ❑ List of references—similar to what was previously submitted electronically <br /> ❑ Project Capability Plan (Attachment C) <br /> ❑ Project Community Values Capability Form (Attachment D) <br /> ❑ Pricing Forms (Attachment E) <br /> ❑ Value Added Plan (Attachment F) <br /> Formatting Requirements <br /> Contractor must follow the formatting requirements of the following Attachments. <br /> ❑ Project Capability Plan (Attachment C) <br /> ❑ Project Community Values Form (Attachment D) <br /> ❑ Value Added Plan (Attachment F) <br /> In order to minimize bias, the attachments identified above must NOT contain any names that can be used to <br /> identify the Contractor.
