Benefits Provided (Continued) <br />Disabled police and firefighter <br />The City continues to pay the employer's contribution toward health coverage for Police or Firefighters <br />disabled in the line of duty per Minnesota Statute 299A.465, until age 65. Dependent coverage is <br />included, if the dependents were covered at the time of the disability. The January 1, 2012 to December <br />31, 2012 monthly premiums paid for Police or Firefighters disabled in the line of duty are: <br />Plan Single Two Person Family <br />Health Partners Choice $ 585 $ 695 $ 901 <br />Health Partners National One $ 696 $ 796 $ 977 <br />Heath Partners Empower High Deductible $ 697 $ 798 $ 980 <br />C. Funding Policy <br />The additional cost of using a blended rate for actives and retirees is currently funded on a pay -as- you -go <br />basis.
