November 19, 1998 James Kielkopf 967 Fremont St. Paul, MN 55106 RE: Request for conditional use permit at 1085 Dionne Dear Mr. Kielkopf: The purpose of this letter is to update you on the status of your request for a conditional use permit to allow the operation of a day care facility at 1085 Dionne Street (Lexington Plaza). The application submitted on Monday, November 9, 1998, has been reviewed and found to be incomplete. The following additional information must be submitted prior to a public hearing being scheduled before the Planning Commission: 1. A completed application form. Responses were not provided for items 8, 9, or 10. 2. An Abstractor's Certificate listing the names and addresses on labels for each owner of land within 350 feet of the boundaries of the subject property. 3. A complete set of plans, including a site plan and building plans with dimensions and/or scale indicated. The plans submitted do not include the proposed interior layout of the facility or the exterior play area. The application packet you submitted including your application fee, is enclosed.
