RO$E Or:J-1MA APAR1Me:NJS PROGRAM SUMMARY '__/ Catholic Charities' St. Rose of Lima Parish will collaborate to provide affordable Housing for single parent mothers and their chiktren and/or mothers who are in the third trimester of pregnancy. The basic approach to families achieving the goal of family stability, self determination, and economic seff sufficiency Will be enhanced through Individualized Goal Attainment Plan and shared responsibilities for various living tasks of the Managed Housing Program. The ptanwill address a plan for training and employment as welf as an opportunity to increase skills in home management. The shared nature of the facility and partnership with St. RO.se of-LIma Parish will also be a major factor in the provision of a supportive. caring environment for mothers and their children. Each woman will pay a minimum of two hundred dollars rent and a minimum of one hundred dollars to a personal savings account. The dollars in the savings account will be retumed to the women to assist with her transition from Managed Housing to independent living. The accompanying goats, objectives, and plan for coordination represent a capsulization of the intended approach. 'v' A. Population to be Served: Single parent mothers over the ages of eighteen who are assessed by intake team to be appropriate and able to succeed in the housing. B. Length of Stay: Six to eighteen months. C. Goal: To establish a collaborative affordable housing.projsct for singJe parent mothers and their children which will address the needs of the whole family to achieve family stability, self-determination, and economic self-sufficiency through a , supportive, educational, and goal attainment environment. D. Objectives: 1. To assure program continuity and successful outcomes Catholic Charities will agree to provide program development, implementation, and program management. St. Rose of Lima parishioners will mutually solicit donations for furnishing individual family dwellings and community space. The two organization wilt meet regularly for planning, problem solving, and evaluation. 2. In order to succesSfully integrate the 'program1 St. Rose of lima parishioners together with designated Catholic Charities staff witl meet with various . neighborhood organizations to review the program, seek recommendations, and gain neighborhood and parish support for the program. 3. In conjunction with Catholic Charities and St Rose of Uma, staff win define the program plan along with policies and procedures which wiUtake into account the fammes need for safety, care, nurturance, goat setting, and structure as ,._" they address and use services for family stability, self-determination, and economic self-sufficiency. ~-------
