Please read and follow these instructions carefully. <br /> <br />PLEASE NOTE: According to Minnesota Statutes, Chapter 53A, regarding the licensing of <br />currency exchanges, both state and local approval of insurance and renewal of a currency exchange <br />license is required. According to statute, all currency exchange locations must be licensed as <br />separate entities. This process may take up to 120 days; therefore, you should return the required <br />forms immediately. <br /> <br />Please sign, date and return this billing with a completed application postmarked before 12:00 midnight <br />September 1, 1999 with a check, cashiers check, or money order in the amount of $500.00 for each <br />location to be renewed. The renewal fee was increased by the 1999 legislative session. Cash cannot be <br />accepted. Make your payment payable to the Minnesota Commerce Department. <br /> <br />Each currency location must file a separate and complete application including all required attachments <br />listed on page 4 of the application. A separate surety bond in the amount of $1 0,000 (on the form <br />provided) must be sent for each licensed location. The bond must be effective January 1, 2000 until <br />December 31,2000. A Power of Attorney form must also be included with the bond. Please note the <br />acknowledgment of individual/partnership/corporation and acknowledgment of surety portion of the bond <br />must be notarized. <br /> <br />If all required forms are not fully completed and returned to our office with payment by September <br />1, 1999, your license will lapse effective December 31, 1999. If your license does lapse, you must <br />reapply for a license using the license application form and paying the new license fees. <br /> <br />ALL THE INFORMATION REQUESTED ABOVE MUST BE ATTACHED TO THE RENEWAL <br />BILLING BEFORE RETURNING THE BILLING TO OUR OFFICE.
