<br />-ADDENDUM- <br />Notice to Applicants for DNR Permit to Change The Course, <br />Current or Cross Section of Public Waters or Public Waters Wetlands <br /> <br />\. <br /> <br />The Department of Natural Resources (DNR) and the Pollution Control Agency (PCA) are woMting together to avoid duplica- <br />tion in state agency permit review of proposed a(.1ivities affecting public waters or public waters wetlands in Minnesota. The attached <br />form was prepared by the DNR and the PCA to minimize your work in contacting state agencies for project approval. <br />Please complete this form by placing an "X" in the appropriate box or boxes. If your project does not involve any of the <br />actions listed on this form, place an "X" in the box after. item 9. . <br />If your project will involve any of the actions listed for boxes 1 through 8. a copy of your DNR public waters work permit application <br />will be forwarded to the PCA for their review. If a separate PCA permit or approval is required. you will be so notified by the PCA. <br /> <br />Place an "X" in the appropriate box(es): <br /> <br />o 1. The project will involve the deposition of asphalt. concrete, cut vegetation or other solid waste as fill material. <br />o 2. The projed will involve excavation of materials from public waters or public waters wetlands through the use of any device <br />which removes Materials by pumping or suction action (i.e. hydraulic dredging). <br />o 3. The projed will involve excavation of more than 1.000 cubic yards of material from the beds .of the following waters: <br />D Lake Pepin of the Mississippi River (River Miles 763 to 787 USCE charts). <br />o Pool #2 in the Mississippi River from Lock & Dam #1 (the Ford Dam) to Lock & Dam #2 (Hastings-River Miles ns to 814 <br />USCE charts). <br />o Minnesota River from Savage to its mouth at the Mississippi River (Mile 644 USCE River chart). <br />D Duluth Superior Harbor and. the 51. Louis Bay area of the S1. Louis River extending to Spirit Lake. <br />D The Fairmont chain of lakes including Budd. Siseton, Hall and Amber Lakes in Martin County. <br />D Albert Lea Lake. Freeborn County. <br />D Warroad River Upper Harbor Area extending from the mouth of the Warroad River to CSAH No. 11. <br />D Red River of the North from Wahpeton to the Canadian Border. . <br />o 4. The projed will involve excavation and/or fill in protected waters for construdion of utility lines carrying any materials <br />except treated water in a liquid or semi-solid state. including but not limited to petroleum or petroleum products, <br />chemicals. sewage or cool slurries. <br />o 5. The project will involve construction of docks, piers or wharves which will involve new fuel handling facilities. <br />o 6. The project will involve new construction. reconstruction or rePair of structures tor generation of hydroelectric power. <br />o 7. The project will involve construction of new ancillary facilities for sanitary sewers, boat pumpouts. on-site waste. <br />D 8.
