Hwy. 36 <br />Our File No.: Min 589 C <br /> <br />Dear Mr. Paschke: <br /> <br />Enclosed please find the above-referenced Conditional Use Permit Application <br />along with a check in the amount of $345.00 for the application fee and city provided <br />property owners list. In lieu of the property owner signing the application, we have also <br />enclosed a Permit Authorization Form from the owner granting Qwest Wireless, LLC <br />permission to seek all necessary permits. <br /> <br />This site will be a parking lot light replacement in the parking lot of Frank's <br />Nursery & Crafts, Inc. With our unipac antenna, it will appear as if the antenna is just a <br />parking lot light although slightly taller than the rest. The light will be replaced at the <br />same height as the existing lights and our ground equipment will be screened by existing <br />bushes at the site.
