GUIDE TO PREPARING <br />SURVEY QUESTIONNAIRE FORM (ATTACHMENT C) <br />And <br />REFERENCE LIST (ATTACHMENT D) <br />OVERVIEW <br />The City of Roseville is implementing a process for Request for Proposals that collects past performance <br />evaluations of firms and their key personnel. This information will be used to assist the City in selecting <br />the best overall valued firm for Services as specified within the scope of service. <br />To assist the City in identifying the past performance of a firm, the following process will be used: <br />1. The firm will prepare a list of clients that will be sent a survey. The general form of the <br />reference list is shown on Attachment D. <br />2. The firm will prepare surveys forms and send to their past and present clients. Use Attachment <br />C for the survey form. (4 minimum- 10 maximum) <br />3. The clients will complete the surveys and send back to the firm. <br />4. The firm will compile and submit all of the surveys and reference list with their RFP. <br />5. The ratings will then be averaged together to obtain a firm's past performance rating. <br />Page 17 <br />
