Attachment D <br />Page 6EMERGENCY CONTINGENCY PLAN <br />REQUIRED REPORTS <br />The Emergency Coordinator will record the time, date, and details of any incident that requires <br />implementing the contingency plan. Within two days after the incident, that person will submit a <br />written report of the incident to Bay West LLC Health and Safety Manager. The Health and <br />Safety Team Manager will review and submit to the proper regulatory offices. This report will <br />include: <br />1.Name, address, and telephone number of the H& S Manager/Team Leader. <br />2.Date, time, and type of incident (e.g., fire, explosion) <br />3.Name and quantity of material involved. <br />4.The extent of injuries, if any. <br />5.An assessment of actual or potential hazards to human health or the environment, where <br />this is applicable. <br />6.Estimated quantity and disposition of recovered material that resulted from the incident. <br />(A sample incident report is located in the attachments) <br />June 2021J120584 <br /> <br />
