12. <br />13. <br />• r. <br />- 2 - <br />For Rezoning Only: <br />A. If property to be rezoned is currently zoned in two or more zoning <br />classifications, set forth the legal description of each such <br />separate zoning classification. <br />B. If proposed rezoning will rezone the property into two or more <br />separate zoning classifications, set forth the legal description of <br />each such separate zoning classification. <br />For Special Use Only: <br />A. City Code Section authorizing special use permit: <br />14. <br />For <br />Vacation of Right -of -Way Only: <br />A. <br />A petition must be submitted, signed by at least 510 of the <br />abutting property owners. The original statement in the petition <br />should be "We, <br />as follows: the undersigned, owners of at least 51% <br />(fifty-one percent) of the <br />property abutting <br />request that this section of right-of-way be vacated because it is. <br />no longer of value to the general public." <br />All property owners signing the petition shall list their address <br />and legal description of their <br />property. <br />15. <br />For <br />Platting Only: <br />A. <br />Number of Lots: <br />B. <br />Subdivision to be known as: <br />C. <br />Engineer: Name Phone <br />D. <br />Land Planner: Name Phone <br />16. <br />For <br />Division of Platted Lot Only: <br />A. <br />Separate legal description for each newly created lot shall be <br />provided. <br />17. <br />For <br />Relocation of Dwelling Only: <br />A. The legal description shall be of the land upon which the building <br />is to be moved. <br />B. The application shall be accompanied by a written consent and <br />approval of at least 500 of the property owners within 250 feet of <br />the relocation. <br />
