<br />ATTACHMENT 5 <br />92 3.Name of property owner, if different from applicant, and signature of property <br />93 owner authorizing use of property for the Temporary Cannabis Event; <br />94 <br />95 4.A copy of the application for a Temporary Cannabis Event Organizer license <br />96 submitted to the Office of Cannabis Management under, and meeting the <br />97 requirements of, Minnesota Statute Section 342.39, subd. 2; <br />98 <br />99 5.A diagram of the Temporary Cannabis Event showing: <br />100 <br />101 a.Location and description of sanitary facilities meeting federal and state <br />102 requirements; <br />103 <br />104 b.Location and description of solid waste disposal facilities meeting state and <br />105 local regulations; <br />106 <br />107 c.Location and description of mobile food vending to be offered at the <br />108 Temporary Cannabis Event; proof of license and permit for vending must be <br />109 submitted at least seven days prior the event and kept on site for immediate <br />110 inspection. <br />111 <br />112 6.Security: In addition to meeting the requirements of Minnesota Statute Section <br />113 342.40, subd. 3, the City may require the applicant employ, at their own expense, <br />114 additional security personnel necessary to protect maximum number of persons <br />115 permitted to attend the event and to preserve order in and around event site as <br />116 determined by the City. No permit shall be issued unless the City police and fire <br />117 departments have approved the security plan. <br />118 <br />119 7.Emergency Plan: Applicant shall provide the City with an emergency plan that <br />120 details procedures for managing or responding to emergencies as required by the <br />121 Minnesota State Fire Code. In addition, for events where there is a possibility for <br />122 more than 1,000 people to congregate, the applicant shall provide trained crowd <br />123 managers. The minimum number of crowd managers shall be established at a <br />124 ratio of one crowd manager to every 250 persons. Where approved by the fire <br />125 code official, the ratio of crowd managers may be permitted to be reduced only <br />126 where the facility is fully equipped with an approved automatic sprinkler system <br />127 or based upon the nature of the event. The emergency plan and amount of crowd <br />128 managers must be approved by the fire department. <br />129 <br />130 8.Parking and Traffic Plan: The City may require the applicant to submit a parking <br />131 and traffic plan, and provide, install, and remove all traffic control equipment if <br />132 necessary.
