L <br /> <br />Page 1c of 3 <br /> <br />Commercial-Account Charges--All hotels, motels, TV retail and service <br />stores, nursing homes, and hospitals. <br /> <br />The maximum monthly service charge per outlet of commercial usage is 50 <br />percent of the residential first-set rate for the same service. <br />Installation charges will be determined on an individual basis after <br />an engineering survey of the facility has been completed. In no event <br />will the charges exceed the standard installation charge .on a per-unit <br />basis. <br /> <br />Bulk-Rate Account Charges--All apartment <br />trailer parks, etc., having five or more <br />the same or contiguous service address <br /> <br />buildings, condominiums; <br />single-family dwellings at <br />with single-point billing. <br /> <br />Bi lling for bulk-rate accounts is based on total units, whether oc- <br />cupied or not. The maximum monthly service charge per outlet for this <br />category is 80 percent of the residential first-set rate for the sarne <br />service. Installation charges will be determined on an individual <br />basis after an engineering survey of the facility has been completed. <br />In no event will the charges exceed the standard installation charge <br />on a per-unit basis. <br /> <br />) <br /> <br />Installation OVer 200 Feet--For installation of service drops longer <br />than 200 feet, the resident will be charged an additional amount for <br />the installation equal to the incremental increase of the cost of time <br />and materials for the portion of the drop over 200 feet. <br /> <br />Miscellaneous <br /> <br />(I) There is no charge for converters for any of the service tiers <br />beyond the installation charges noted on page 1b of 3. <br />(2) There is no charge to subscribers for cable-related maintenance <br />and repair calls. This includes Universal service. <br />(3) There will be no charge for the first drop to governm.ent, library, <br />educational and non-profit users of the institutional network. <br />Commercial rates will vary depending on service level. <br />(4) Teleprompter of the North Suburbs will offer pay-per-view services <br />at the lowest possible rate pending the conclusion of contract <br />negotiations with vendors. <br />(5) Remote control available on Tiers II, III and IV at $1.50 per <br />month. <br /> <br />) <br />
