Proiect Questions <br />Please answer the questions #I-7 on separate pages (3 page limit, 10 point font). Refer to Appendix D of the <br />Guidelines & Information to see how projects will be evaluated. Be sure to give comprehensive information including <br />all activities. <br />1. Organizationlagency Description — Provide a brief description of the applicant's organizationlagency <br />including its qualifications to complete this project. <br />2. Project Site Description — Describe the site of the project in detail along with special characteristics such as <br />soil composition, existing vegetation, etc. <br />3. Project Purpose — In what ways will your project meet the needs of your community forest? Explain how the <br />project will benefit and enhance the community and why you feel your project should receive funding over <br />others. <br />4. Personnel � Describe the key staff, contractors, volunteers andlor partners and how they contribute to the <br />project. Attach letters of support showing commitment of key partners. <br />5_ Project Coals & Objectives — What are the project's goals and objectives for each grant category applying <br />for? Who will be responsible for these activities and when will they occur? Include any educational outreach <br />efforts, work with volunteers and especially the technical plans and practices. <br />6. Maintenance Plan -- Upon completion, what are your plans for maintenance and managing the components of <br />your project in the future? Include how, when and by whom activities will be accomplished for at least 3-years <br />after project completion. <br />7. Long Term Community Forestry Plan — Whatis the communitylorganization doing to build capacity towards <br />long-term management of this project or developing an urban and community forestry program independent of <br />state funding? <br />Forest Health Manaaement Plan <br />ALL Forest Health applicants must submit a 3-year management plan with their application (3 page limit for plan, 1 <br />page limit for 8'/2" x 11" map). See qaqe 13 in the Guidelines for rectuirements.
