If the cost of a benefit purchased by you with Premium Conversion increase (or <br />decreases) during the plan year and your are required to make a corresponding change to <br />your election, the Employer may, on a reasonable and consistent basis, automatically <br />make a prospective increase (or decrease) in your elections. <br />The Employer may add to the list of changes in circumstances that constitute Status <br />Changes, consistent with the law relating to such Status Changes. <br />TYPE OF PLAN <br />This is a flexible benefit plan that permits participants to choose among more than one <br />benefit. It is classified as a"cafeteria plan" for federal income tax purposes. <br />BECOMING ELIGIBLE TO PARTICIPATE IN THE PLAN <br />A new Eligible Employee may elect to become a participant in the Plan on the first day <br />the Employee becomes an Eligible Employee, and beginning after he or she becomes an Eligible <br />Employee by satisfying the participation conditions. Thereafter, Eligible Employees may <br />become participants on the first day of a Plan Year. <br />PARTICIPATION CONDITIONS <br />As a condition to participation in the Plan and to receiving reimbursement benefits under <br />this Plan, you must: <br />(1) Eligible Employees that have elected to participate in the Employer's health, <br />dental plan and/or life insurance plan(s) shall be deemed to have elected to <br />participate in the premium portion of this plan, and to have the employee portion <br />of the premium for coverage under the Employer's health, dental plan and/or life <br />insurance plan(s) paid for on a pre-tax basis, unless written notification, on <br />Employer forms, is supplied to the Employer, stating that the employee does not <br />wish to participate in the premium portion of this plan and that the employee <br />portion of the premium for coverage under the Employer's health, dental plan <br />and/or life insurance plan(s) should be paid for on an after-tax basis . Eligible <br />Employees shall have the right to waive participation in the Employer's health, <br />dental plan and/or life insurance plan(s), and take any contributions they would <br />have made as taxable income; <br />6 <br />
