Article 3. Eligibility and Participation Conditions <br />Section 3.1 Participation Conditions. As a condition to participation and receipt of <br />benefits under this Plan, an Eligible Employee agrees to: <br />(a) To comply with the provisions in 3.2 (a); <br />(b) Furnish to the Employer the application to participate provided for <br />in Section 3.2 (b); <br />(c) Designate a portion of his/her Compensation as Pay Conversion <br />Contributions in accordance with the provisions of Section 4.2; <br />(d) Observe all rules and regulations implementing this Plan; <br />(e) Consent to inquiries by the Employer with respect to any <br />physician, hospital, or other provider of Medical Care or other services involved <br />in a claim under this Plan; and <br />(� Submit to the Employer, or such other agent as the Employer may <br />designate, all reports, bills, and other information which the Employer may <br />reasonably require. <br />15 City of Roseville MPD <br />
