AGREEMENT FOR <br />EMERGENCY MEDICAL TECHNICIAN <br />SERVICES <br />THIS AGREEMENT, is entered into this _ dav of November 2004, by and between <br />the City of Roseville, a political suhdti��ti�i�n �"� ii��"� and the Roseville Youth Basketball <br />Association, a non-profit corporation ("RYBA") <br />WHERAS, the City Fire Department has certified Emergency Medical Technicians <br />('BMT") on staff; <br />WHEREAS, RYBA acknowledges that the City does not provide paramedic, ambulance, <br />or medical transportation and understands that those services are not part of this <br />Agreement; <br />WHEREAS, RYBA and the City desire to enter into this Agreement for the purpose of <br />providing EMT coverage for RYBA youth basketball games at an agreedprice. <br />NOW THEREFORE, IN CONSIDERATION OF the foregoing and the mutual <br />promises and agreements contained herein, the parties agree as follows: <br />Resvonsibilities of the Citv. <br />(a� Provide stand-by EMT services at RYBA youth basketball games. <br />(b� Provided medical supplies consistent with an EMT level of inedical care. <br />(�� Issue invoice no later than 30 days after EMT services are provided under <br />this Agreement. <br />�. Resvonsibilities of RYBA. <br />(a� Provide City a list of dates and locations of RYBA youth basketball games <br />no later than 14 days before the first game. <br />(b� Maintain a log of the medical and/or allergic conditions that RYBA knows <br />its participants have and have that log available at all RYBA youth <br />basketball games. <br />�_ Pavment. RYBA shall pay the City at a rate of $15.00 per hour for the EMT <br />services. Payment shall be made in one lump sum payable no later than 30 days <br />after the completion of services. <br />�_ Term. The term of this Agreement shall be from the date first stated above <br />until October 31,2005. This Agreement shall be automaticallyextended from <br />EMT AGREEMENT <br />Page 1 of 2 <br />
