As part of the grant award process, these are <br />the steps you must follow: <br />1. Your local commission needs to meet and select a preferred vendor(s) to assist in your <br />project. <br />2. Your local commission's chairperson must notify the Minnesota Department of Human <br />Rights, (the Department) in writing, of your acceptance to the funds and the terms of the <br />grant no later than March 31, 2009. You can send all correspondence to the attention <br />of Mario A. Hernandez. The grant agreement you must complete is attached. <br />3. In the grant agreement, you need to include a brief description of the event/training, <br />when and where the training will take place, the contract information for the <br />contractor/vendor(s) providing the services and the contact information for your <br />commission's "fiscal coordinator" (if you submitted a grant request you already selected <br />a fiscal coordinator, please send the updated contact information). <br />4. The Department will contact the contractor/vendor(s) you selected. The Department will <br />• process all contracts and payments directly with your contractor. In case any problems <br />arise, you will receive communication from James Kirkpatrick, Deputy Commissioner for <br />the Department, or his designee, explaining what problems exist with either the vendor <br />or the services to be contracted. <br />5. Upon completion of the services and or training, your "fiscal coordinator" will certify that <br />the training was held as contracted. The fiscal coordinator will include a list of the local <br />commissioners who attended/participated in the training/event. If your project was a <br />strategic planning session, please send a copy of your finished product. <br />6. Upon receipt of your fiscal coordinator's certification of training/event, and an invoice <br />from the contractor/vendor, the Department will pay the contractor/vendor for services <br />rendered. <br />Note: In selecting acontractor/vendor, we recommend that you look at more than one <br />possible contractor/vendor before making your selection. <br />If your commission submitted more than one request during this first round of grants and the <br />project was not selected, please re-apply at a later deadline for another round of mini- <br />grants. Make sure you follow the guidelines for grant submission.
