REQUEST FOR COUNCIL ACTION <br />DATE: 9/30/02 <br />ITEM NO.; ;� �. <br />Department Approval: 1�'Ianager Reviewed: Agenda Section: <br />/ % ��� / <br />Item Description: Approve Change Order No. 2, 2002 Contract B <br />Background: 2000 Contract B includes paving necessary to rehabilitate the following road segments. <br />2002 Contract B- Re-Bid <br />Neighborhood 16: Lydia Ave, Chatsworth St to Cul-de-sac <br />Lydia Drive, Lydia Ave to Chatsworth St <br />Neighborhood 24: Walnut St, County Road C to Rose Place <br />N Hwy 36 Service Drive, St Croix St to Long Lake Road <br />Rosegate, N Hwy 36 Service Drive to Cleveland Ave <br />Neighborhood 25: Oakcrest Ave, Prior Ave to Fairview Ave <br />The proposed Change Order is for subgrade correction in the Lydia neighborhood, as well as for <br />additional curb replacement along Rosegate. We encountered poor soils during our street rehabilitation <br />of Lydia. Our initial quantities accounted for a subgrade excavation depth of 6 inches. It was <br />necessary to increase this to 12 inches in some areas, for an average of 10 inches in this neighborhood. <br />The additional curb was removed and replaced as a result of working with property owners on <br />driveway reconstruction as well as needing more of the existing curb rehabilitated than was originally <br />estimated. The Change Order increases the contract by $42,678.25. This is a 6% increase to the <br />original contract price of $578,135.80. <br />Policy Implications: The proposed Change order is for over $5000 and requires Council approval. <br />This work has already been completed because of project timing, we needed to proceed with the <br />correction during the Council break. Waiting for approval of this change order would have delayed <br />completion of the project by one month. <br />Financial Implications: The cost for Change Order No. 2, $42,678.25 will be funded using <br />Infl'astruCtul'e funds. <br />REC01�'Ii�'IENDED COUNCIL ACTION: <br />Motion approving Change Order No. 2 for 2000 Contract B. <br />Prepared by: Debra Bloom <br />
